We invite you to participate in our survey concerning your overall satisfaction in

canteen’s overall performance. We are conducting this survey for the purpose of knowing if you
are satisfied with the canteen’s duty in terms of service, price, and products and also to know if
the things that might be improved in the canteen’s overall performance.

Your participation in this survey is voluntary. You may agree or disagree all throughout
but we need your truthful judgement. The questionnaire is composed of ten questions in total.
