Good day!

This letter will intend to inform you about the error regarding the misnomer of the agent’s code. The
incautiousness of my action allows this concern from existing.

This problem arose dated September 10, 2020, and this concern subsisted when the application of Mr.
Lemuel Dela Cruz Doromal, that was installed on August 20, 2020, Account No. 691394085, 0814 Silver
St San Rafael Village Marfori Heights Davao City, with a plan of 50MBps, was encoded under the code of
Ms. Michel Alpuerto (507049), an agent, that supposedly should be encoded to Ms. Irish Mancito
(506953).

I will hold myself responsible for this mistake and hoping that it would not happen again in the future. I
hope for your understanding and verdict of this problem. For clarification of the incident, I will gladly
answer your queries upon your request.
