Event Purpose
The Mini Forum on FSMS started because we would like to promote FSMS in Cambodia as
well as organizations who have been certified food safety standard certification. Also, we would
like to make our attendees and all people in Cambodia know about food safety standard values.
We decided the best way to solve this problem is to conduct training on this field for many
times, and to create annule events relating to food safety standard, which we have done 2 times
already. Since then, it has helped increasing human resources in field of food safety standard,
FSMS certificate is known more, and people are aware of food safety certificate sticking on
products' label.

We have come a long way, but we still need your support. Your generous contribution will help
us extend our journey by your funds.

Sponsorship Terms & Conditions

This Mini Forum on FSMS Sponsorship Contract (the “Contract") states the terms and conditions that govern the contractual agreement

between sponsor company having its principal place of business , and FSG event host (the "Host") who agrees to be bound by this

Agreement.

WHEREAS, the Host is conducting Mini Forum on FSMS, on 01 December 2019; and

WHEREAS, the Sponsor agrees to sponsor the Event according to the terms and conditions herein.

NOW, THEREFORE, In consideration of the mutual covenants and promises made by the parties hereto, the Company and the Sponsor

(individually, each a “Party" and collectively, the “Parties") covenant and agree as follows:

Contribution

The total contribution provided by the Sponsor is ------------------------, to be paid in full by MM/DD/YY

Term/Termination

This Contract will terminate automatically upon completion of milestone.

Changes/Cancellations

Any changes made to this Contract must be made in writing to all parties. If the Event is canceled, funds are turned back.
