SKYBroadband offers high-speed internet connectivity that is more than what the usual dsl and wireless
connections can offer. Through Sky’s coaxial facilities, faster upload and download speeds can be
generated making it a more efficient tool in supporting your business initiatives. This service also offers a
range of options to suit your access needs.

TERMS AND CONDITIONS

1. Installation Period - within 3 to 5 working days from awarding, and submission of all required
documents and availability of the client.

2. Requirements needed – In order for the service order to be processed, Client should submit the
following:

a. Submit the following requirements upon acceptance of this proposal :

  Signed Proposal Conforme (this form)
 Application for Data Services Form (to be provided)
 Business Permit or DTI Registration (Latest)
 Proof of Billing any of the following (Meralco, Manila Water, Telephone Bills)
 Valid Company ID with signature of the authorize HR

b. Ensure the following are ready and available from the time of installation and while the
service is being provided:
 Adequate power supply including standby genset and UPS/AVR unit to ensure 24-
hour uninterrupted operations
 Provision of inter-facility cable runs, entrance conduits, and proper grounding for all
equipment attached to Sky’s facilities
 In-house wiring. (In the absence of in-house wiring, Sky may provision or the
necessary wiring chargeable to the Customer)

c. Service Acceptance - Upon service installation and activation.

The service will be billed every month. The bills will be released depending on the billing cycle with a due
date after 21 days from the billing cut off. A 2% interest will be applied per month on late payments. Non-
payment of the two months subscription will result in service suspension.
d. Cancellation of Order - Should customer cancels the order before the service activation, customer will
reimburse to Sky the actual expenses incurred in the installation.

e. Contract Term and Pre-termination Charges - This agreement will be commencing on the acceptance
date and will be automatically renewed for twelve (12) months unless a written notice of termination is
received from the customer (30) days prior to its expiration.

PRE-TERM FEE : ( 3 x Monthly Subscription Fee)

f. Validity of Proposal – This proposal shall be valid for a period of 30 days from the date of receipt by the
customer.

g. Confidentiality Statement - This proposal contains proprietary information and is released for the sole use
of the customer. Disclosure to third parties is permitted only with written authorization of Sky.

h. Customer’s Acceptance - Notwithstanding the signing of this proposal conforme, Sky reserves the right to
amend its quoted investment summary due to additional equipment that may be necessary for a successful
installation. Likewise, customer has the right to defer or cancel its order for the affected circuit(s) as a
result of this amendment.

We accept the rates, terms and conditions as quoted in this document. This serves as our approval.
