SCRIPT ROLEPAY HEALTH ASSESSMENT

Nurse : Hallo Good Morning, I’m Nurse Nur Ida, what’s your name?
Patient : Hallo Nurse, my name is Farizka.
Nurse How old are you Mrs. Farizka?
Patient : I’m 22 years old.
Nurse : Ok, Mrs. Farizka. You are so pale. Would you tell me you’ve
been admitted to hospital today?
Patient : I have been eaten some spicy meals yesterday, now I have
stomachache and I suffer from diarrhea?
Nurse : How long have you been having this pain and diarrhea?
Patient : The stomachache started many years ago. So far, more than
10 months now. My stomach hurts after eating some meals. I
forgot yesterday that I had stomachache, so I ate more spicy
meals and immediately I suffered from diarrhea.
Nurse : Have you got some medicine before you go to the hospital?
Patient : I haven’t, emmm………just drink a cup of less sugar tea
Nurse : Let’s go to the bathroom please!
Patient : No…no, I haven’t pain now Nurse, I can hold on.
Nurse : What do you feel like? Is it stabbing or sore?
Patient : A sore of stomach Nurse.
Nurse : How would you describe the intensity of pain, on a scale of
one to ten?
Patient : Well, I’d say the pain about a three scale of one to ten, it’s not
really bad. It’s make me tired.
Nurse : How long does the pain last when you get it?
Patient : The pain comes and go. Sometimes I hardly feel anything.
Other times, it can last up 20 minutes or more
Nurse : How about your diarrhea? How many time have you had
bowel movement?
Patient : It occourred approximately 5-6 times
Nurse : Is there a type of food that seems to cause stronger pain than
other types?
Patient : Em….. heavy food like spicy noodle, ghost noodle, nasi
goreng☹
Nurse : What’s the ghost noodle?
Patient : The name of noodle is in My city.
Nurse : Oh hahaha…. I just to know
Does the pain travel any other parts of your body, chest,
shoulder or back? Or does it remain arround the stomach
area?
Patient : No, it just hurts here.
Nurse : Ok, wait a minute please. I’ll make the report of your condition
to The Doctor.
