 CONVERSATION BETWEEN NURSE AND PATIENT

Nurse : Hello Ms. Angel, is there anything I can help?

Patient : Yes, I need help.
Nurse : What do you feel right now?
Patient : I feel pain in the area of my hand.
Nurse : Could you explain to me the chronology until it happened like this?
Patient : When I wanted to transfer hot water, I accidentally tripped, and finally hot
water spilled on my hands.
Nurse : What has been done to treat this wound?
Patient : I have tried giving Bioderm Cream Gentamycin Sulphate 0,1%. But never
heal.
Nurse : Ok. First, can you roll up your sleeves? I’ll measure your blood pressure.
Patient : Of course, I can.
Nurse : Your blood pressure is 110/80 mmHg. Now I’m going to measure your heart
rate and you respiratory frequence. Your heart rate is 70 and you respiratory
frequence is 22.
Patient : Ok.
Nurse : Do you have a history of previous illness?
Patient : I didn’t have previous medical history. This is the first time I was
hospitalized.
Nurse : Is there a hereditary disease in the family?
Patient : High blood pressure.
Nurse : Now, I will measure the level of pain you feel, you can answer what I asked,
do you understand?
Patient : Well, I understand.
Nurse : If you were asked to give the numbers 1 until 10, what number of pain did
you feel?
Patient : I think the pain I feel is on a scale of 6.
Nurse : When did your pain occur?
Patient : When I move my hand, it feels very painful.
Nurse : Can you tell about the pain you feel like?
Patient : The pain was like slicing.
Nurse : Does the pain persist or continously?
Patient : The pain was continuous.
Nurse : Next, I will check the head area, its okay?
Patient : Okay.
Nurse : The head area appears clean and there are no injuries. Next, I will carry out an
assessment of your body using inspection techniques, palpation, percussion
 and auscultation.
Patient : Okay.
Nurse : Now, I will take a blood sample, and later a laboratory examination will be
carried out.
Patient : Of course, you can.

Nurse : I have given fluids, so that you don’t lack fluids due to these burns. And I will
teach you deep breath relaxation techniques to relieve pain, are you willing?
Patient : Well, I’m willing.

Nurse : Hello Ms. Angel, What do you feel today?

Patient : I already feel better than yesterday.
Nurse : Ok, now I will change the bandage on your burn, are you willing?
Patient : Well, I’m willing.
Nurse : This dressing must be change so that the wound can avoid infection.
Patient : How many times does this burn need to be cleaned?
Nurse : While in the hospital, every morning I will check and clean your burns. Is it
still very painful at this time?
Patient : Not as painful as yesterday
Nurse : I had replaced the wound bandage, for later you can take the medicine that
was given.
Patient : Thankyou for your help.
Nurse : You’re welcome, you can rest now.
Patient : Okay.
