DEPARTMENT ORDER NO. 174

sequence below)

With attached Proof of Compliance with substantial capital
requirement as defined in Section 3 (L).
 5M paid up capital stock/shares -for corporation,
partnership & cooperative.
 5M net worth - for single proprietorship..
B. Any of the following:
Certified true copy of the Certificate of Registration from SEC,
along with the Articles of Incorporation.
Certified of DTI Registration or Certification.
Certified true copy of the Certificate of Registration from
CDA.
Certified copy of Registration from the DOLE if applicant is
a union.
C. Certified true copy of License or Business/Mayor’s Permit
issued by the Local Government Unit where the contractor
operates.
D. Copy of duly audited financial statement, for Corporation or
Partnership, Cooperative or Union; or copy of the latest
Income Tax Return (ITR), for sole proprietorship
E. Sworn disclosure that the registrant, its officers and owners
or principal stockholders or any one of them has not been
operating or previously operating as a contractor under
different business name or entity or with pending cases of
violation of these Rules and/or labor standards, or with a
cancelled registration. In case any of the foregoing has a
pending case, a copy of the complaint and the latest status
of the case shall be attached.
F. Certified Listing with proof of ownership or lease contract of
facilities, tools, equipment, premises implements,
machineries and work premises that are actually and
directly used by the contractor in the performance or
completion of the specific job or work contracted out.
G. Photo of the office and premises where the contractor holds
office.
H. Registration fee of P100,000.00.
Additional requirements per D. O. 150-16: (For Security
Agencies)
Certificate of membership issued by PADPOA or other
associations recognized by the PNP-CSG-SOSIA
License to operate issued by PNP-CSG-SOCIA
Certificate of pending or no pending labor standards
violation cace/s issued by NLRC and DOLE. This applies to
new applicants who have already operated prior to its
application for registration

Copies of updated supporting requirements pursuant to
Section 21 of D.O. 174 (B to E, listed above) shall be attached
including the following:
 Certificate of membership and proof of payments of SSS,
BIR, ECC, Pag-Ibig, Philhealth and loan payments and
contributions for the last 3 years
*Certificate of No Pending; or No Pending Labor Standards
Violation case/s with the NLRC and DOLE.The pendency of the
case will not prejudice the renewal of the registration, unless
there is a finding of labor standards by the DOLE Regional
Director
Registration fee of P100,000.00.
