I. GENERAL ACTIVITIES:

a. Attended traditional Monday flag raising ceremony at Tabuk City hall

grounds.
b. Processing of Business permits at the Business One-Stop-Shop located at
the City Hall Building Lobby.

c. Inspections of Establishments and Issuance of FSIC/NTC/NTCV.

d. Collection, deposit and reporting of fire code revenues.

II. OTHER ACTIVITIES:

d. Polishing and cleaning of station and premises by outgoing Duty

personnel.

e. Warm-up and BLOWBAGETS check of fire truck by the duty driver pump
operator or assigned duty personnel

f. Proper turn-over of duties and responsibilities by outgoing and Incoming

Duty personnel.

III. SIGNIFICANT ACTIVITES DURING THE PERIOD COVERED:

050848H JUNE Rosenbaeur fire truck driven by FO3 Aguac together with FO1 Laureta and NUP
2020 Dormagon proceeded to St. Joseph Parish church to conduct flushing activity.
11 1645H JUNE Rosenbauer fire truck driven by FO3 Dulansi with crew SFO1 Apalla, SFO1
2020 Bartolome, and NUP Ramirez responded to a reported vehicular fire incident at
Balagtas St., Dagupan, Tabuk City.
23 0945H JUNE Daeyang FT driven by FO1 Aliño accompanied by SFO1 Apalla conducted
2020 follow-up fire investigation of a fire incident happened on 26 March 2020 at Ipil,
Tabuk City, Kalinga.
24 0825H JUNE Daeayag FT driven by FO1 Aliñ accompanied by SFO4 Ella and FO1 Soga-agn
2020 went to Brgy. Ipil to get copies of brgy. Settlement agreement of a fire incident
that transpired last March 2020.
24 0945H JUNE Rosenbauer FT driven by FO3 Julio T Dukansi Jr with crew-SFO1 Apalla and
2020 SFO1 Danag proceeded to an ocular inspection and investigation of a structural
fire that transpired at Brgy. Bantay, Tabuk City last June 22, 2020.
270900H JUNE Rosenbauer FT driven by FO3 Jan Hendrick G Aguac accompanied by SFO4
2020 Ella and CFM was put on standby at BFP-KPO during the visit of the Regional
Director.
