Data D- 45 year old male, admitted at ED, anxious, appears pale and lethargic, with IVF 1L
PNSS @4 gtts/min at 900cc level, with the pain score of 5, with the vital signs of T-37.4,
HR- 108, RR- 16, BP- 122/82, O2sat- 100%

Actions A- Assesses patient’s general condition


- Monitored VS and charted
-Assessed location and intensity of pain
- Monitored and regulated IVF at desired rate
-Administered medication as ordered
-Instruct patient to do diversional activities like reading magazines and listening to
music
-Assisted patient to change position every 2hours
-Teach patient to do breathing technique pattern
-Promote proper ventilation and a therapeutic environment
-Advised SO to always stay on the patient bedside
-Provided opportunity for patient to rest

Response R-Seen patient comfortably lying on bed and verbalized that pain can be tolerated.
