INSTRUCTIONS:
1. Examine carefully the compacted SLM (SLM for Quarter 3 is separate from Quarter 4) for compliance to language standards indicated in the
four (4) Classifications indicated below.

2. List down all identified language errors by giving a brief description and by citing the quarter, lesson number, and the specific page where it is
found. Give your recommendations to correct these errors in the appropriate column.

1. Classify the identified language error by indicating “1” in the appropriate column.

2. Add all the identified language errors in each classification and write the total number of errors in the provided space found at the bottom of the
classification column.

3. Indicate the rating for each classification by referring to the Equivalent Rating of Total Errors per Classification.

6. To compute for the weighted rating for each for Classification, use the following formula:

(Equivalent Rating of Total Errors) / 5 x Assigned Weight for the Classification

7. Add all the computed weighted ratings for each classification to get the overall rating for this area of evaluation.

8. To PASS in this area of evaluation, the set of SLMs shall garner 100% total overall rating. Total overall rating of 60% to 99% is categorized as
CONDITIONALLY PASSED.
