PREVENTIVE ALERT SYSTEM IN SCHOOLS (PASS) upon School Entry and during

Module Distribution

1. The Preventive Alert System in Schools (PASS), based on DepEd Order No. 34, s. 2003,
is a systematic relay of information on a child’s, teacher’s or other personnel’s state of
health to appropriate personnel and/or agencies in the locality.

2. A health declaration form is given to the parent or guardian and all teaching and non-
teaching personnel upon entering from the guard on duty. Parents and guardians
must fill out this form at the school gym. The school personnel fill out only once and
must submit the said form at the Administration Office. Individuals with other
transactions must fill out and leave the form at the guardhouse.

3. Temperature must also be checked by the guard. If the temperature exceeds 37.5 °C,
he/she must proceed to the screening room for further health assessment. If the
temperature does not normalize and other symptoms exist, he/she must proceed to
the isolation room. The attending school health personnel must immediately inform
the Barangay COVID Task Force.

4. The parent/guardian will then proceed to his/her child’s classroom and submit the
health declaration form to the class adviser followed by the claiming of modules.
He/she must follow the route of entry and exit as designated by the school personnel.
The entire procedure repeats during the claiming of modules for the 3 remaining
quarters.
