AFFIDAVIT LOSS

I, CRISTELA B. LAZO, of legal age Filipino and with residence address at #36 19 Ave., Ice
Plant, San Roque, Cubao, Quezon City 1109, after having sworn to in accordance with law do
hereby deposes and state, that:

1. That I am the owner/possessor of Company ID issued by Corporate Apparel, ATM payroll

account (BDO), Company issued uniform.

2. That on November 3, 2020, I surrender the abovementioned items to company through

LBC courier service, however, the same did not receive by the above company. That I
conduct further inquiry to the LBC but it appeared that the items were lost in their
custody.

3. Considering above factual circumstances, to locate the abovementioned items would be

rendered in futility.

4. That I am executing this affidavit to attest to the truth of the foregoing instrument and
for any legal purpose and intents this may serve.
