to monitor clients for any physiological changes such as monitoring vitals and observing clients while using the
tilt table and would execute the correct plan of action if necessary. I am sure to ask for assistance as necessary

Self Mid-Term — Summer J. Demeuse I require no supervision/guidance and am consistently putting clients' needs above my own. I haven't had any
situations that require taking action for ethical/legal concerns but am confident that I would be able to address
them appropriately. Without supervision/guidance, I adhere by all policies and procedures of the facility as well
as legal practice standards. I utilize time management strategies to give myself time to efficiently meet client's
needs and provide services to the best of my ability.

Self Mid-Term — Summer J. Demeuse I am getting a lot more opportunities for interdisciplinary communication at this facility where I am able to
improve my abilities to select appropriate information relayed to other disciplines and communicate efficiently.
As I am attending weekly inter- and intradisciplinary staff meetings, I will require less guidance from my CI in
future interactions with staff members. Communicating with clients has required some guidance from my CI
with clients' who have more complex impairments. I am requiring less guidance with further experience with
clients who have difficulty communicating. I am always attentive during communication with others to
facilitate positive conversations and relationships with staff and clients.

am able to communicate without supervision with clients including with clients who have difficulties

regardless of complexity. As complexity increases or with unfamiliar situations, I require minimal
supervision/guidance to select interventions that the client will be able to perform yet are challenged. With little
experience in this setting, I benefit from occasional guidance for clinical expertise and patient preferences from
my CI. As I am getting to know clients more and gaining experience, I am requiring less guidance. I have read
and evaluated a few published articles to apply to clinical practice but could be more consistent with utilizing
research to aid in clinical decision making. I am aware and recognize my limitations of current knowledge,
theory, and judgement in patient management.

Self Mid-Term — Summer J. Demeuse I have reviewed medical history for a majority of the clients' who I am currently working with which include
history prior to/after an accident, progression of medical status through hospital stay, reports from hospitals
prior to admission to the facility, and initial evaluations from multiple disciplines. I have had the opportunity to
perform screening elements such as reviewing medical history with clients, gross strength/ROM, balance,
vitals, muscle tone, sensation, etc. I was able to perform and interpret these tests and measurements
appropriately and accurately with minimal supervision.

9. Patient Management – Examination


Self Mid-Term — Summer J. Demeuse I have been able to perform examination tasks on a few clients due to the limited amount of new clients at the
facility. I am independent at obtaining subjective history from clients and utilizing other data from previous
reports to select examination components that are appropriate for the individual. Tests and measures that I have
conducted at this site include: aerobic capacity (6MWT), gait, assisted locomotion, balance (Berg), joint
integrity/mobility, muscle performance, tone, pain, posture, ROM, and sensation. Within this setting, I require
minimal guidance occasionally with tests and measures for this population that I am unfamiliar with/lack
experience. As I gain experience and confidence in my skills to conduct these components, I am requiring less
frequent supervision. Efficiency in conducting examination components and confidence in my skills and
findings are areas for improvement.

efficiently performing reexamination components with many clients since the midterm evaluation with little to

Self Mid-Term — Summer J. Demeuse I am able to establish a PT diagnosis and appropriate interventions such as decreased ROM, strength,
coordination, balance, etc. With a lack of experience/clinical expertise in this setting, I require
supervision/guidance to develop a prognosis and discharge status for clients. I am able to estimate contributing
factors that may impact the effectiveness of interventions, but would benefit from supervision/guidance due to
lack of experience. I require intermittent supervision/guidance when determining the appropriate intensity of
exercise/interventions when dealing with clients of higher complexity. Some examples of these complexities in
this setting would include: clients who are easily frustrated by progressing exercises or making them too
challenging, clients who are reluctant to performing suggested exercises or through challenging progressions,
etc.

Self Final — Summer J. Demeuse With more experience, I am becoming more efficient and requiring less guidance when determining the
appropriate intensity of interventions with clients of higher complexities. Determining prognosis is challenging
for me with clients in this setting due to the wide range of functional gains that are made in individuals who
have had a brain injury. I require min-mod guidance when determining prognosis but would require less with
increased experience in this setting.

12. Patient Management – Plan of Care


Self Mid-Term — Summer J. Demeuse As this setting transitioned to a new documentation system, my CI and I have discussed specific goals that are
appropriate at this facility. I have not been independent with creating a PT plan of care, but have collaborated
with my CI with designing a POC and modifications based off the patient's response to therapy. My CI and I
have also discussed factors in this setting such as differences between when improvements or maintenance in
functional outcomes would be appropriate. Considering contributing factors, I have had collaborative
conversations with my CI about modifying the plan of care (discharge, independent exercise program,
supervised exercise program, etc). I have been able to collaborate with another PT about examination findings
and the appropriate exercise prescription (i.e. independent exercise program vs supervised). I have been able to
progress and modify components of the plan of care such as therapeutic interventions based off of patient
responses.

Self Final — Summer J. Demeuse I am independent with selecting interventions based on evidence and patient preferences. I am able to progress
and modify the plan of care based of patient responses, for example increasing frequency of shoulder
strengthening for a client or discharging a client due to goals being met and ability to perform a HEP. Same as
previous with establishing goals and POC while collaborating and learning how these are written in this setting
with limited new clients.

Self Mid-Term — Summer J. Demeuse I am able to perform most interventions listed safely and effectively independently. In this setting, I was
requiring assistance at the beginning providing patient-related instructions due to inexperience with
communication and comprehension impairments that some of the clients have. As I gain more experience, I am
becoming more independent and efficient. I require guidance/supervision for the prescription, application, and
fabrication of interventions such as orthotic and supportive device/equipment due to lack of experience and
knowledge in this area. I am able to independently perform interventions within the plan of care and follow
established guidelines. I am regularly assessing patient responses to interventions and adjusting accordingly
such as progressions, regressions, or any physiological S&S that require modifying the intervention (i.e. vital
monitoring). I am able to provide rationale to my CI for interventions selected for clients and have productive
discussions.

impairments that I am improving my abilities to identify and communicate appropriately to provide education. I
am able to provide education to clients such as an HEP for a client and the importance of adherence for desired
outcomes. I am always striving to ensure understanding of the client and their ongoing program to facilitate
positive relationships and a mutual understanding of desired outcomes. As I gain more experience with this
population, I am becoming more independent at educational interventions. I haven't had the opportunity to
educate caregivers or family members at this facility yet.

Self Mid-Term — Summer J. Demeuse At this facility, I am not involved in any scheduling duties. I am able to coordinate PT with other services such
as providing appropriate guarding for safety while bringing a client to the next therapy session or assisting them
inside from transportation vehicles. I do not submit billing at this facility, but I discuss and give rationale for the
units that I would be billing while providing documentation. I required guidance initially when learning the
differences in billing from my experience in past settings, but require minimal to no guidance now with more
experience.

Self Final — Summer J. Demeuse I have independently scheduled clients with the new documentation system at this facility. I consistently use
time effectively while performing documentation tasks at lunch or in the case where a client cancels, etc. I have
been responsible for recording billing charges on a sheet for the head of the department to submit/review
without supervision. At a department meeting, I have given suggestions for equipment or supplies that would be
beneficial to have for sessions.
