Self Mid-Term — Emma Vaitkevicius I am able to maintain a safe working environment and able to recognize changes with patients. Demonstrates
awareness of contraindications and precautions to ensure patient safety. Use proper handling techniques when
caring for patients and understands facility safety. I need to improve my quality and consistency with
comprehensive understanding of patient precautions and contraindication to ensure patient safety during

Self Final — Emma Vaitkevicius I continued to provide safe environments for my patients. I always utilized gait belts when necessary, made sure
that patients were at safe heights on plinths and maintain a clean environment. I ensured that my patients felt
comfortable and able to express personal feeling and emotions as well.

comment below with explanation/justification. Safety is one of Emma's strongest areas, both for herself and her
patients, as should be expected of someone who has worked in a SNF. Emma does an excellent job of providing
appropriate guarding for patients; providing education on biomechanics, safe transfers, and AD utilization; as well
as doing an above average job of protecting her own body. Remember that you want to do this for 30+ years, so
make sure you're taking care of your biomechanics the same way you would when you have the body of a 60 year
old. :) Unrestricted rating: 14/20

CI Final — Richard LaFave Emma utilizes safety equipment with good understanding and in appropriate situation. She is acutely aware of her
surrounding and ensure a safe environment for the clients she worked with. She was thorough in her explaining and
education of clients.

Self Final — Emma Vaitkevicius I felt that I demonstrated initiative by arriving on time, offering assistance and continue to seek out learning
opportunities. I was professional toward all patient and remained professional toward staff as well. I exhibited
caring, compassion, and empathy toward all patients and contributed to positive work environment. I continued to

comment below with explanation/justification. Emma does an incredible job of blending professional behavior with
warm compassionate care, a difficult needle to thread. I tell students to utilize the "grandma rule", to treat all
patients the way you would want your grandma to be cared for and Emma does a wonderful job of exemplifying
that. She provides communication both verbal and nonverbal that is appropriate and for the most part confident.
She is timely and always professionally dressed, and beats me to the clinic more days than not. She also does a
wonderful job of protecting patient confidentiality and modesty and accepts feedback readily. Unrestricted rating:

CI Final — Richard LaFave Emma demonstrated good interaction with staff, colleagues and clients. She always conducted herself in a
professional manner in various situation and environments. client interactions were handled with integrity and
professionalism, especially when educating and answering of question. there wee a couple time where she was
unsure of herself, she was still able to conduct herself in a professional manner.

Self Mid-Term — Emma Vaitkevicius I am consistent with maintaining patient confidentiality, abiding by policies regarding HIPPA and OSHA, maintain

consistent supervision/guidance with responsibilities and steps to remedy and prevent errors.

Self Final — Emma Vaitkevicius I continued to place patient needs above myself and identify my error and take responsibility for then. I took steps
to increase my knowledge, whether that was researching or consulting with my CI to ensure I was providing the
best care. As I continued with my clinical experience I felt more confident in my abilities to provide care without
error while under supervision/guidance. I maintained patient confidentiality, adhering to legal practice, and
displaced generosity. I continued to strive to provide client services that I felt went beyond my expected standard
of practice.

her age. She is always accurate and truthful in her communication, does an excellent job of maintaining patient
confidentiality and readily takes radical responsibility for her areas of growth. This is not an area for concern.

Self Mid-Term — Emma Vaitkevicius I am able to communicate verbally and nonverbally with patients and initiate communication. I am developing my
skills in communicating in terms that both patients and clinical staff can understand, making sure to remain
consistent. I seek guidance with communicating more complex topics with patients and how to adjust style of
communication based on the audience. I am improving my active listening skills with patients to ensure that they
feel understood.

Self Final — Emma Vaitkevicius I felt that my communication with the patient has improved significantly. I was able to develop my skills with
communication (not using complex terminology) to ensure my patients were understanding. I would continue to
seek guidance with building my communication skills and understanding when I need assistance or education from
my CI to help. I utilized my active listening skills to ensure that the patients felt understand and to ensure that
treatment was based off of their needs/goals.

comment below with explanation/justification. This is probably one of Emma's biggest areas for growth, for all the
right reasons. When she's conveying black and white information to patients she is confident and ready to answer
any question that has been posed to her by a patient. Where she struggles is with communicating "in the grey".
Emma's biggest challenge with communication is her confidence with communicating her *original* ideas related
to diagnosis/prognosis and patient education/recommendations to patients. Different clinical instructors deal with
this area differently, and I err on the side of support for the student: Emma, unless you're grievously off-target, I'll
always support your communication with patients. We'll work on developing a strategy to help you with this, until
you have what my wife prays for: "Give me the confidence of a mediocre white male". Unrestricted rating: 8/20

cultural biases in order to provide better care for patients.

comment below with explanation/justification. Emma performs wonderfully in my rather diverse clinic when it
comes to cultural competence, especially for a student raised in the homogeneity that is the UP. I have a clinic
population that at times is minority majority, which can be a huge adjustment from the expectations of a normal
outpatient ortho clinical population. She also does well working with what are traditionally biased patient
populations like work comp. One of the things I love about Central's program is that they don't teach population
specific POC until after a student's second clinical, so students don't come with too much inherent bias with work
comp patients. Often students from other schools have been taught wrongly either implicitly or explicitly that part
of their job is to catch malingerers which teaches them it's the norm. Nothing could be further from the case, and
Emma understands that well. Unrestricted rating: 15/20

Self Mid-Term — Emma Vaitkevicius With guidance from my CI, I have been focusing on my ability to self-evaluate and understand my strength and
limitations. I seek supervision and guidance with limitation, including communication and treatment for high
complexity patients. I am continuing to work on and understand my limitations, including discussing clinical and
professional growth, accepting responsibility, continue professional learning, providing and receive feedback from
peers.

Self Final — Emma Vaitkevicius I felt that I had a lot of growth when it came to my professional development. I continued to utilize the guidance of
my CI to evaluate my abilities and understand my strengths and limitations. I was able to work with complex
patients with confidence and continued to research and utilize resources to understand/obtain prognosis for
patients. I would seek out additional learning experiences from other clinicians to continue to develop my
professional development with different patients. I felt comfortable discussing and providing feedback and goals
as well to ensure I was continuing to grow as a clinical.

Self Mid-Term — Emma Vaitkevicius I have limitation with knowledge and education surrounding complex cases. I require supervision/guidance for
making clinical decisions, recognizing plan of care and interventions, implementing changes. I am continuing to
assess my limitations and work on my ability to develop effective plan of cares and interventions. I am working on
these skills by making sure I have a reasoning and am able to the discuss why I am choosing and intervention.

Self Final — Emma Vaitkevicius I felt that I grew in my ability to present logical rationale for clinical decision making. I gained experience, utilized
resources, and CI guidance to make clinical decisions. I recognized when aspects of plan of care and interventions
were ineffective and how to adapt/modify them to benefit the patient. I continued to select interventions based on
the best evidence and the experience that I had gained. I assessed patient responses to interventions and
understood when to adapt/progress patients. I continued to recognize my limitations in my knowledge and where I
need to continue to grow.

explanation/justification. Emma is performing exactly where I would expect a student in their 4th trimester to be
performing. She does incredibly well with the foundations of clinical care and progression as well as
diagnosis/assessment with simple and common conditions. She sometimes begins to struggle with reasoning
exactly where she should at her stage of learning, when patients present with multifactorial difficulties and
complex cases. She is at the stage in her program where we are still teaching students diseases and disorders as
well as prognosis and treatment in a relative vacuum. I have no doubt that this will continue to improve immensely
as she grows through this clinical and through her academic/professional career.

CI Final — Richard LaFave she is still formulating her flow for evaluations and patient's POC. I noticed the improvements made during her
rotation and she will continue to grow as she become more ingrained in the profession. She has a lot of the pieces
in place she will learn to formulate them into an ebon flow for efficiency when treating her clientele.

8. Patient Management – Screening

Self Mid-Term — Emma Vaitkevicius When it comes to patient screening, this is a skill that I feel needs the most improvement. I am confident with
reviewing patient history and gaining the proper subjective information to determine what objective measures I
need to assess. I feel that I am able to appropriately conduct test and measures but am still working on my
sequencing of taking objective measures and interpreting all of the results.

Self Final — Emma Vaitkevicius I felt that my screening abilities and skills improved. I was able to utilize more test and measures confidently to
evaluate my patients. I reviewed the patient's PMH, precautions, and interventions to have a more comprehensive
understanding. I would perform systems reviews. I continued to gain confidence in conducting test and measures
and interpreting test and measures appropriately. I feel that I need to work on my cohesiveness, sequence when
selecting appropriate tests and performing my screening.

Comments for additional information). Emma does an excellent job of selecting the appropriate anthropomorphic
tests and measures for patients based on their medical history and chief complaint. In her short time with us she
has learned to utilize a handheld dynamometer and explored several additional tools in order to best assess a
patient's needs.

CI Final — Richard LaFave overall does a good job of reviewing the material and screening each patient. She utilized FOTO and our patient
portal to review gather and implement strategies to screen patients.

9. Patient Management – Examination

Self Mid-Term — Emma Vaitkevicius The skills that I feel confident in are my ability to obtain history for patients. I am able to utilize this history and help
integrate it so that I can determine the test and measures I want to conduct. I am still gaining experience with all
the different tests and measures. The aspects I still need to work on are my sequencing of tests and measure to
optimize efficiency and choosing the appropriate test for complex patients.

Self Final — Emma Vaitkevicius I continued to develop collecting my subjective to include relevant and important information for the patient. I
continued to utilize information from history and other data when developing plans of care, interventions, and
treatments. I selected evidence based test and measure that were relevant to patients history/ chief complaint, and
screening. I felt that my sequencing improved significantly as I got to practice more and develope my skills.

Comments for additional information). Emma does a great job of using her training in PT school as well as her
experience as a CNA to quickly and effortlessly evaluate the basics of patient function and be ready to flow into
determining what she wants to examine in greater detail. She hasn't had the chance to evaluate everything in her
brief time with us, but she has had an opportunity to see how patients work with assistive devices, patients for
work reintegration, assessed most of the basic functional performance (e.g.: motor function, range of motion, pain,
etc).

CI Final — Richard LaFave Is starting to get a nice flow to her examination. I recommended continued some test and measure aren't left or
she doesn't hyper focused of specifics and could adjust based of the patient interaction data collections of testing.

Self Mid-Term — Emma Vaitkevicius After obtaining the appropriate subjecting and objective information, I feel confident in evaluating the information
and interpreting what it means. I am able to make clinical judgements based off this information and get an idea of
where I want to begin when it comes to treatment. I still need to work on my efficiency and evaluating more
complex patients.

Self Final — Emma Vaitkevicius My confidence and abilities in synthesizing examination data to identify impairments, limitation and quality of life
has improved. I improved my efficiency and evaluating complex patients. My ability to make clinical judgements
based on the data I have collected has improved. I feel that I am able to reach clinical decisions efficiently based
on the knowledge and experience I have gained.

Comments for additional information). Emma has learned in her brief time with us how to synthesize the data from
examinations for and start to parse out the important information from a sea of data. She has also grown in
figuring out where to go next, but does still require "Ok, you have X, and Y, have you checked Z?" prompting on more
complex cases to guide her decision-making. Once she has the relevant data, she benefits from a vote of
confidence to say "Ok, now that we have X, Y, and Z, what do these things all get you towards?" with complex cases,
however if she has all the relevant information, she is very capable of hoping right into diagnosis/prognosis with
simple cases.

CI Final — Richard LaFave There was nice grow throughout the clinic when conducting evaluations. she developed a more succinct flow to her
examinations while collecting data from her patient and test and measure. will continue to grow but should focus
on hypothesis and potential alternative based on her test and measures. learn to perform all testing in positions
before moving to the next. she has started to develop this and will continue as she participates in future rotations.
She has a bright future ahead of her.

11. Patient Management – Diagnosis and Prognosis

Self Mid-Term — Emma Vaitkevicius I feel that my ability to develop a diagnosis and prognosis is another skill that requires more improvement. When it
comes to simple cases, I am able to integrate the information I have obtained and develop a clear and thorough
diagnosis. I am still struggling with complex cases, especially spine pathologies and neurological pathologies. I am
working on my skills of explaining prognosis to patients because I am still gaining experience and seeing what a
typical prognosis is for a patient with a specific diagnosis. I am able to identify how contributing factors affect
prognosis and use reasearch and literature to fill in my gaps of knowledge.

Self Final — Emma Vaitkevicius I felt that I have improved greatly with my ability to establish diagnosis and differential diagnosis for both complex
and simple cases. I feel that I am more able to determine a diagnosis that is congruent with pathology, impairment,
functional limitation, and disability. I have a more comprehensive understanding of prognosis for patient with
specific diagnosis and understanding rehab potential based off of patient presentation.

information to arrive at a diagnosis and has become increasingly effective at it. Where she struggles is conveying
the diagnosis and prognosis to patients with confidence. The more reps you get with this, the stronger root will be.

CI Final — Richard LaFave does a nice job of formulating and differentially diagnosis low complexity cases based on data collection from test
and measures and patient reporting. with mod to higher level complexity, she will continue to grow, learn to
decipher the information to formulate different differential diagnosis.

12. Patient Management – Plan of Care

Self Mid-Term — Emma Vaitkevicius I am confident in my ability to establish a beginning plan of care for patients, since I do not have much experience
with working with patient for a long period, I am still working on my ability to progress them through treatment. I am
able to base the plan of care off of the evidence available and patient preference. I am able to see and understand
when a patient is not able to perform a specific intervention and how they must be modified. I feel that I do a good
job at assessing risk and benefits with the patients and following up with them. I need to continue to work on my
ability to obtain goals and develop functional goals for the patients.

Self Final — Emma Vaitkevicius I feel that I have continued to improve on my ability to develop a plan of care. Since gaining more knowledge and
experience, I have a better understanding of effective intervention and plan of cares for patients. I have been able
to see outcomes and how to modify patient intervention to improve plan of care. I also feel confident in developing
a plan of care that is specific and goal oriented to the patients needs. I feel that I continue to do a good job at
assessing risk and benefits with the patients and following up with them. I have improved my ability to develop and
obtain goals that are functional and specific to the patient.

confidence with communication for patients. You know how to do this, and your interventions are well thought out.
You just have to build on consistently communicating the plan to patients with confidence.

CI Final — Richard LaFave Emma was comfortable in treating patients and keeping a good clinic flow going. this will come with time and
experience as there were a couple times the clinic became very busy and may have been a bit overwhelming.
Emma did a nice job of recognizes this and had my assistance. where she can grow is utilizing support staff. She
was able to facilitate the clients progression with good thought process.

13. Patient Management – Procedural Interventions

Self Mid-Term — Emma Vaitkevicius I feel confident in performing interventions safely, effectively, and efficiently. I make sure that all interventions are
performed based off the plan of care and utilizing the patients goals when developing interventions. I use literature
and research when following guidelines and when it is safe to progress a patient. I need to work on sequencing of
treatment and making sure they are fluid for the patients. When a patient is unable to perform an intervention or
has a particular diagnosis, I am able to adjust treatment accordingly. I make sure to incorporate the concepts of
self-efficacy and am consistently considering health and wellness associated with the plan of care

Self Final — Emma Vaitkevicius I continued to improve on my ability to perform interventions safely, effectively, and efficiently. I provided
interventions that were specific to the patient and to the patients goals. I continued to research the literature, utilize
CI knowledge and experience and my own knowledge to develop and utilize interventions. I felt that my sequencing
improved significantly with treatment and making sure the treatment was fluid for the patient. I was able to modify
and adjust interventions while incorporates concept of self-efficacy to promote health and safety for the patient.

Comments for additional information). Procedural interventions is the area where Emma's direct patient care
shines brightest. She has a warm, open, and inherently trustworthy way of interacting with patients that makes
them feel heard when they're hurting. Simultaneously, she does a good job of challenging people who are further
along and able to tolerate intense exercise as they progress towards return to sport.

CI Final — Richard LaFave found ways to effectively adapt each client's treatment not only by their needs but also their level of symptoms for
each treatment session. she wasn't afraid to try new things to accommodate and make sure patient's needs are
met. this may be one of her strongest areas.

Self Mid-Term — Emma Vaitkevicius I feel confident in my abilities to identify and establish priorities when it comes to patients education. I need to
improve on my ability to assess and identify a patients learning styles and barriers to learning. I am consistently
working on applying my knowledge and skills to identify further problems and recommending solutions during
treatment.I am struggling with not talking in medical terms, which I am working on everyday to become more
consistent at. I also working on my ability to ensure the patient has an understand of the treatment and diagnosis.

Self Final — Emma Vaitkevicius My ability to provide education for the patient improved significantly. I improved on my skills of assessing and
identifying a patients learning style and barriers to learning. I continued to work and apply my knowledge and skills
to identify further problems and recommending solutions during treatment. I was able to work with complex
patients that helped me develop these skills and improve my skills at adapting. I improved at not speaking in
medical terms to ensure that my patients had complete understanding. I got more confident applying education for
the patient to promote health and wellness. I understood where my limitations were and when to utilize the help of
my CI to ensure the patient understood.

Comments for additional information). This is another area where Emma can shine when she's confident. If you ask
Lisa, I was beaming, grinning from ear to ear listening to you give education to your eval on one of your last days at
Lansing Delta. When your confidence is lacking people can definitely hear it, so that's an area for growth, but you do
know the material. You just need to work on sharing it.

CI Final — Richard LaFave did an excellent job educating patient with adaptive equipment, safety, and exercises. Her training with gait, ADLs
and IADLs. areas to improve explaining pathologies and treatment strategies fostering patient's understanding
during initial evaluations.

Self Mid-Term — Emma Vaitkevicius I am feeling confident in my ability to select the relevant information required for documenting treatment for the
day. I am still working on understanding each component required in the EHR and how to navigate it. I am able to
utilize terminology that describes the care that was provided that day. I need to improve my efficiency when
documenting so that it is done in a timely and legible matter.

Self Final — Emma Vaitkevicius I continued to develop my skills at writing cohesive notes that were completed in a timely manner. I am continuing
to understand each component required in EHR and how to navigate it. I am developing my terminology that
describes the care that was provided.

weeks. She has built her documentation up with the help of using templates to improve flow and to help synthesize

and evaluations to make sure they are efficient and concise. I need to improve my ability to assess patient when it

CI Mid-Term — Jake R. Lipper t Emma's biggest area for development is in confidence with communication. Remember Emma, you know this stuff
like the back of your hand. If Michael Barnes can have confidence in writing a prescription, you can have
confidence in providing a PT diagnosis and exercise prescription. Don't let them see you sweat, fake it till you make
it, whatever mantra you want to use, the truth is, you know the material, you know the patient care, you just need to

CI Mid-Term — Jake R. Lipper t 1. Caseload is variable throughout the year with a yearly average weekly caseload of 55 visits being considered a
goal caseload. 2. Emma is performing well above the Advanced Beginner expectations of the clinical rotation. She
has nearly achieved the caseload requirement for Intermediate and is working towards those requirements with
regard to supervision. Emma is capable of managing ~46% caseload of a new graduate with ~86% supervision
with simple cases and ~87% supervision with new/complex cases at this time. Absolutely incredible performance,
especially for only being in her 4th trimester.

CI Final — Richard LaFave did a masterful job engraining herself into a busier than normal caseload. She met many of these cases head on
facility patient's progress and experience. She was handling ~ 50% of the caseload, supervision to her digression
but we always discussed. well beyond the normal first clinical rotation student.
