STEP 1: OPEN THE BROWSER/GOOGLE

STEP 2: Search Google Forms (make sure that your Gmail account already signed in)

STEP 3: Click the second site which is the Google Forms: Free Online Surveys for Personal Use
STEP 4: Over here under Personal click (Go to Google Forms)

STEP 5: This is the site where you can create a Google Form, so over here there is a blank where
you can create your own templates. Also, there are few ready-made templates for Blank quiz,
Exit Ticket, Assessment and Worksheet.
STEP 6: Click Blank Quiz (Since I am going to make a quiz as an example)

STEP 7: Put the title of the quiz in (Form Tile) and Put the Directions in the (Form Description)
STEP 8: Provide the space for their name, course & year and date in the (Untitled Question)
(Click the plus sign in the right part for add question)

STEP 9: Put the first question in (Untitled Question)


STEP 10: Put the choices of answer in (option 1 then add option or add other)
(For the answers, we can choose other style such as short answer, paragraphs, multiple choice,
check boxes, dropdown, file upload, linear scale, multiple choice grid and tick box grid, Time and
Date) But in my example I choose multiple choice.

STEP 11: Click the (Answer Key) to choose the correct answer and put the points. Then click
(Done). The process will be the same so work on it until the last number.
STEP 12: Click the paint symbol in the upper right if you wanted to put an image in the header
and also change the theme color, background color and font style.

STEP 13: Click the setting on the upper right

STEP 14: Click the following in the (General) part.


STEP 15: Click (Presentation) then put a confirmation message

STEP 16: Click (Quizzes) then click the (Make this a quiz) Then click (Save)
STEP 17: Click (Send)

STEP 18: Click Send via Link then Copy (Send the link to students for them to answer)
STEP 19: Open the link then its all set, students may answer already

STEP 20: After the students answering the quiz, you may click Reponses to see the results and
all done
