RESOLUTION No. 2021 — 08

RESOLUTION ADOPTING AND SUPPORTING E.O. #5 S. 2021 OF MAYOR JONAS C. CORTES

“CREATING THE COVID-19 VACCINE BOARD OF MANDAUE CITY”

WHEREAS, E.O. #5 S. 2021 of Mayor Jonas C. Cortes has created the COVID-19 Vaccine Board of
Mandaue City;

WHEREAS, the executive order has provided the powers and functions of the board as may be
necessary, proper or incidental to their attainment of the COVID-19 vaccination program of the city;

WHEREAS, Barangay Centro, adopts and supports the Vaccine Board of Mandaue City for the
proper implementation of its vaccination program.

NOW, THEREFORE, on motion of Kagawad Trinidad C. Catarig, duly seconded by all Members
present.

RESOLVED, as it is hereby resolved, to adopt and support E.O. #5 S. 2021 of Mayor Jonas C. Cortes
“creating the COVID-19 Vaccine Board of Mandaue City".

RESOLVED FURTHER, to furnish copies of this resolution to the Mayor Jonas C. Cortes and City
Health Office for their informafion.

RESOLVED FINALLY that the resolution remains effective and subsisting and has not been
amended, resolved or superseded.

APPROVED and ADOPTED this 7th day of April 2021, Mandaue City
