Sacral OMM

Sacral Diagnosis
 Seated Flexion Test  Which side?
o Side that is restricted is side of dysfunction
 Patient Lays Prone
o Palpate the Sacral Sulci
 Think where Back Dimples Are
o Palpate the ILA’s
 Walk Down with Palm until Drop off
o Spring Test
 Place Hand on Sacrum and Push Down
 + Spring Test (No Springing)  No Flexion
 Think L/R, R/L Extension
 - Spring Test (Springing)  Flexion
 Think R/R, L/L, Flexion
Physiologic Dysfunctions Muslce Energy (L/L, R/R)
 UP UP UP or DOWN DOWN DOWN
o Chest, Axis, Force
 L/L Variations
o Chest Up, Left Side Up, Patient Force Up (Top Leg)
o Chest Down, Left Side Down, Patient Force Down (Top Leg)
 R/R Variations
o Chest Up, Right Side Up, Patient Force Up (Top Leg)
o Chest Down, Right Side Down, Patient Force Down (Top Leg)
 Always Stabilize the Sacrum when Treating
 Reassess
o Palpate the Sacral Sulci
o Palpate the ILA’s
o Spring Test

Non – Physiologic Dysfunctions Muscle Energy (L/R, R/L)

 UP DOWN DOWN or DOWN UP UP
o Chest, Axis, Force
 L/R Variations
o Chest Up, Right Side Down, Patient Force Down (Top Leg)
o Chest Down, Right Side Up, Patient Force Up (Top Leg)
 R/L Variations
o Chest Up, Left Side Down, Patient Force Up (Top Leg)
o Chest Down, Left Side Down, Patient Force Down (Top Leg)
 Always Stabilize the Sacrum when Treating
 Reassess
o Palpate the Sacral Sulci
o Palpate the ILA’s
o Spring Test
Flexion Dysfunction Muscle Energy
 Negative Spring Test  Flexion is Intact (Left/Left, Right/Right, Flexion Dysfunction)
o Sacral Sulcus is Deep
o ILA is Shallow
 Technique  Respiratory Assist Mechanism focused on pushing down on ILA
 Loosen Sacrum by Abduction and Adduction
 During Inhalation and Exhalation  Push down on ILA to Resist Flexion
and Promote Extension
 Reassess
o Palpate the Sacral Sulci
o Palpate the ILA’s
o Spring Test

Extension Dysfunction Muscle Energy

 Positive Spring Test  Extension Dominates (Left/Right, Right/Left, Extension
Dysfunction)
o Sacral Sulcus in Shallow
o ILA is Deep
 Technique  Respiratory Assist Mechanism focused on Pushing down on Sacral Sulcus
 Loosen Sacrum by Adduction and Abduction
 Arch Your Back
 Push Down on Sacral Sulcus
 During Inhalation and Exhalation  Push down on Sacral Sulcus to Resist
Extension and Promote Flexion
 Reassess
o Palpate the Sacral Sulci
o Palpate the ILA’s
o Spring Test
