AFFIDAVIT OF UNDERTAKING

I, LUCILO MANALO y MAGBOJOS, Filipino, of legal age, married and residing

at Blk. 28, Lot 3, Zone 1, AFP Housing Barangay Bulihan, Silang, Cavite, after having
been duly sworn to in accordance with law do hereby depose and say that:

1. I am the Accused in Criminal Case No. B-2019-981 for RECKLESS

IMPRUDENCE RESULTING IN HOMICIDE WITH ABANDONMENT, pending before
Regional Trial Court, Branch 89, City of Bacoor, Cavite;

2. As such, the bail recommended for my provisional liberty is pegged

at THIRTY THOUSAND PESOS (Php 30,000.00) and I am ready to post the said bail
in cash;

3. Upon granting of my bail, I hereby undertake to appear before this

Honorable Court whenever so required by it or by the Rules of Court;

4. Failure on my part to appear at the trial or hearing without justification

despite due notice to me shall deemed an express waiver of my right to be present on the
date specified in the notice and that in such case, the trial may proceed in absencia;

5. Whenever I desire to travel abroad during the pendency of the

proceedings, I shall promptly move for permission from the court. I further undertake to
submit myself for the execution of final judgment;

6. I executed this affidavit to attest to the truth and veracity of the foregoing
statements and for all legal intents and purposes this may serve.

expiring on 2023-10-31, presenting the above Affidavit of Undertaking, who is personally
known to me and identified by me through competent evidence of identity as defined by the
Notarial Rules as the person who sign this affidavit and in my presence taken an oath or
