In compliance to the National Deployment and Covid-19 Vaccination Program and

Hospital Advisory No. 2021-004, “Mandatory Orientation of ALL BGHMC Employees
including Job Orders, Human Resources for Health (HRH) Staff, outSourced Employees
(Care Best and Security), Interns and Affiliates” dated February 1, 2021

INSTRUCTIONS:
Options:
1. Group orientation may be conducted following minimum health standards in a well
ventilated and spacious area.
2. Individual orientation through online may be conducted for as long as the employee is
instructed properly and monitored if he/ she actually viewed the recorded videos.

Orientation of Employees on the Covid-19 Vaccination.

1. Let the employee have his/her pre-test taken (you may access link at the 1st Google
Doc entitled Pre and Post Test).
2. Choose the right folder in the Drive: Medical or Non-medical
3. The modules are arranged as follows:
a. Module 1: Overview of the Covid-19 Vaccination Plan
b. Module 2: Covid-19 Vaccination Survey
c. Module 3: Understanding the Covid-19 Vaccination
d. Module 4: Adverse Event Following Immunization (not included in the Non
Medical)
e. Module 5: BGHMC Covid-19 Plan
f. Module 6: Vaccination and Registration Set-up
4. After viewing all the modules in the drive, the employee will now answer the post test
and evaluation.
5. The department secretary or person in charge shall make sure that everyone will sign
the attendance sheet. (Name, Sex, Position, Department, Signature) The Attendance
Sheet shall be noted by the HEPLink assigned in the area and Department Head.
6. The Attendance Sheet (scanned) shall be Submitted on or before February 14, 2021 via
online to the Public Health Unit Office by the Department Secretary.
7. The HEPLink together with the Department Head shall make sure that 100% of
employees in their unit/ department/ section are oriented.
8. Kindly write your questions. An OPEN FORUM via WebEx (link will be sent soon) will be
scheduled on February 11, 2021 at 9:00 to 11:00 AM for the Medical and 1:00 to 3:00
PM for the Non Medical.
