FOR EXISTING SUBSCRIBER

1. I understand and agree that this proposed transfer shall not take effect nor be binding on GLOBE TELECOM until after this approval by
GLOBE TELECOM. I shall, as a precondition to the approval of this proposed transfer of ownership, settle any and all unbilled and outstanding
bills, which accrued prior to the date of GLOBE TELECOM’s approval of the proposed transfer of ownership in accordance to the terms and
conditions set by GLOBE TELECOM. I understand and agree that my failure to settle any outstanding bills or make arrangements for the same
to the satisfaction of GLOBE TELECOM shall be a ground for the disapproval of the proposed transfer and the termination of this account, and
I hold GLOBE TELECOM free and harmless from any liability for such disapproval.
2. I further acknowledge and agree that this transfer of ownership is without prejudice to GLOBE TELECOM’s billing for any and all local NDD,
IDD, incoming collect and operator-assisted calls and other call charges which may be hereafter be discovered as having accrued before the
effective date of this transfer, all of which I shall settle in accordance to terms and conditions set by GLOBE TELECOM.
3. I agree that the effectivity of this proposed transfer of ownership as against GLOBE TELECOM is further contingent on GLOBE TELECOM’s
approving the new subscriber in accordance to its existing subscription, identification and credit requirements. GLOBE TELECOM has the sole
discretion to approve or disapprove any proposed transfer and to terminate the account if such proposed transfer is not approved. I hereby hold
GLOBE TELECOM free from any liability for such approval.

approving the new subscriber in accordance to its existing subscription, identification and credit requirements. GLOBE TELECOM has the sole

GLOBE TELECOM free from any liability for such approval.
3. I agree, if transfer of ownership was approved through policy exemption, to continue and be bound to the terms and conditions of whatever sales
promo signed by the existing subscriber until the lapsing period stated in the promo and that there will be no termination and/or downgrading
within the specified time period as stated by the promo.
4. I hereby confirm that I have read and understood the Terms and Conditions of our Subscriber Agreement found at globe.com.ph/terms-and-
conditions and that I shall strictly comply, abide and consent by these terms and conditions and any future amendments thereto.
5. I hereby confirm that I have read and understood the Globe’s Privacy Policy as stated in https://www.globe.com.ph/privacy-policy.html and that I
shall strictly comply, abide and consent by these terms and conditions and any future amendments thereto.
