Deep Perineal Pouch

• Space between two


fascial layers ( superior
& Inferior fascia of
urogenital diaphragm)
• Extends across the
pubic arch
• Closed on all sides
Boundaries
• Above – Superior fascia
of Urogenital diaphragm
• Below – Inferior fascia of
urogenital diaphragm
(perineal membrane).
• In front – transverse
perineal ligament
• Behind – fusion of
superior & inferior fascial
layers.
• Laterally – inner surface
of ischio-pubic rami

• Membranous urethra
• Bulbo-urethral glands in
male
• Vagina behind urethra in
female.
• Muscles :

Superior fascia of

• Blends with perineal
membrane anteriorly.

• Continuous with obturator


fascia on each side.

• Reflected on sides of
urethra in midline and
continuous with inf.

• Seperates the superficial
& deep perineal pouch
• Extends along pubic arch,
▲ in shape, apex in front.
• In males crus of penis is
attached to inf. surface of
perineal membrane.
• Attached to ischio-pubic
rami on each side
• Thickened in the front to
form tr. perineal lig.
• Posterioly attached to
perineal body.

superficial & deep fibers

• Both fibers encircle the urethra

• Superficial fibers from pubic


arch, trans. Per. lig attached to
perineal body

• Deep fibers arise from ischio-


pubic rami encircle the urethra
and continuous with opposite
muscle.
Actions of urogenital
diaphragm
• supports prostate in male,
bladder in female.
• constricts vagina
• fixes the perineal body
• sphincter urethra exerts
voluntary control of
micturition.
• Rupture of urethra below
perineal membrane -
Extravasation of urine

• Fibro muscular node receives


muscles in 3 strata.

• Levator ani – deep stratum.

• Sphincter urethra & deep trans.

Pudendal Canal

• Fascial canal along the


lateral wall of Ischio-rectal
fossa.

• Extends from lesser


sciatic foramen to postr.
limit of Deep. Per. Pouch.
