Introduction

Blue Badge Nursing Home prepared this security incident report for the theft and loss incident that occurred last
September 9, 2025. The facility’s electronic safe located inside the main o ce was cracked open, resulting in the
loss of $500,000.00 in cash.

Objectives

The objectives of this incident report were:


1. Determine the security lapses of the facility
2. Agree on the appropriate actions to take to address and avoid the incident to occur again

Incident Information

Johannson Hendricks, the head of nances, went to the main o ce to look for some documents when he noticed
that the electronic safe was opened. He informed the chief of security and went to le a report for the theft and
loss.

Methodology

The security team of the facility gathered facts about the incident from interviewed resident nurses, patients, and
staff. Copies of the security protocols, currently followed by everyone living and working in the facility, were also
collected to conduct the investigation.

Findings

1. All entry and exit points of the main o ce were not forced open, therefore, the theft was most likely working
within the facility or had a duplicate of the facility’s keys.
2. According to the resident janitor, he saw a man go out of the main o ce at around 8 in the evening where
most of the staff are already out.

Conclusion

The stated incident happened due to the failure of the facility to observe adequate screening and security
measures.

Recommendations

1. All keys in the facility should be changed.


2. Additional CCTV cameras shall be placed in and out of the facility.
