Weekly Mass Toolbox Talk – Life Saving Rules

Purpose & Objective of LSR:


 Life Saving Rules strengthen organizational EHSS culture and clearly demonstrate
that management will not tolerate unsafe behaviour in the workplace.
 LSR help compliance to Safe Work Practices and It support the Behavior Based
Safety.
 While all EHSS procedures/ systems are important, experiences show that failure to
follow certain “EHSS Rules” are more likely to result in serious injury or death, etc.

Commitment:

 There are clear and consistent Life-Saving Rules.


 Everyone should comply with these Rules.
 Required investigation for each potential non-compliance.
 Holding people accountable: management, supervisors, staff, contractors and sub-
contractors.

Summary:

Deviation from Life Saving Rules can result in fatality and/ or life-altering injuries. The
actions arising as a consequence of not following LSRs will depend on the circumstances that
allowed the incident to occur. Incidents due to in-effective training or lack of full
comprehension may be addressed by procedural changes or improved training/ validation.
Whereas, incidents arising from willful motivation to consciously deviate from the LSR
procedure may lead to a disciplinary outcome on accordance with established site procedures.

Imminent Dangers:

1 Lift overhead loads on windy conditions exceeding the allowable limits (32 km/h).
2 Cross barrier-controlling area with suspended loads without authorization.
3 Use lifting certificate without proper permit.
4 Work is carried out without a valid permit.
5 Issue, receive, and/or close a permit without job-site verification.
6 Work in confined space without confined space entry work permit.
7 Work in confined space without gas testing.
8 Work in confined space without standby man.
Work in an elevated platform / places (>1.8 meter) without guardrails and not using full

body harness.
10 Working on scaffold with red tag.
11 Work on expired Scaffold platform.
12 No energy lock out system.
13 Working electrical circuits without approval.
14 Work on EHSS critical equipment without proper authorization.
15 Tamper with or compromise safety systems.
16 No Banksmen / Flagmen when reversing.
17 Open Process Equipment without wearing proper PPE.
Perform unauthorized removal or tampering with hazardous energy control devices
