SWORN STATEMENT OF JO1 HARVIE B SISA , GIVEN TO JO3 CLAY Z

GUTIERREZ JMP, INVESTIGATOR- MANDAUE CITY JAIL MALE DORMITORY,

TAKEN AT THE ADMIN OFFICE ON 30 JULY 2019.

Preliminary: JO1 Harvey B Sisa, you are being informed that there is an ongoing
investigation being conducted in relation to the shooting incident that happened on or
about 1042H of 29 July 2019 at the Roof Top of Mandaue City Jail- Male Dormitory,
that took the life of one PDL and other one PDL wounded. Being on duty at the time of
the incident, your statement is necessary in order to shed light into this investigation. Are
you willing to give your statement in your own freewill and volition?
Answer: Yes sir.

Q1 Please state your name and other personal circumstances?

A1 I am JO1 Harvey B Sisa, of legal age, married, resident of 183-A Salvador Ext.,
Labangon Cebu City and a bona fide member of BJMP assigned in Mandaue City
Jail Male Dormitory.

Q2: When were you assigned at Mandaue City Jail Male Dormitory?
A2: Since May 04, 2018

Q3 : At around 10420H of 29 July 2019, where were you at that time?

A3 : I was on duty detailed at the Roof Top of this facility.

Q4: Did you recall if there was any unusual incident that happened during that time?
A4 : Yes sir.

Q5: What was that incident all about?

A5 : There was a basketball tournament held at the roof top, and in the middle of
the game one PDL in the person of Jade Saladaga entered the basketball court and
with no apparent reason shoot PDL Romy Ramas who was playing basketball then. PDL
Ramas was hit at the head and fall to the ground.at the and PDL Ray Harold Saladaga
who was
playing basketball that time using a .38 caliiber revolver.

Q6: What happened to PDL Ramas after he was shot by PDL Saladaga.
A6: PDL Ramas was hit at the .

Question: Were you alone at the Desk Officers’ station during the said time?
Answer : No sir, there were other duty personnel who were with me at the time sitting
inside the D.O.s’ post.

Question: Can you still recall who were those duty personnel you were referring to at that
time?
Answer : I could not remember but I am sure almost all duty personnel of the Main
Building and the Drug Offenders’ Building at that time sir.

Question: Why did that happen that inmate Arnel Saladaga were able to go to the chapel
area before the said inmate could manage to bolt out of this center?
Answer: During that time sir and as usual all the cells were open for all the inmates to
play basketball and volleyball at the basketball court wherein the chapel also is
nearby situated.

Question: When was the time it come into your knowledge that there was one inmate
who manage to escape from this center?
Answer: It was when I heard a gun burst sir and followed by a shout in visayan dialect
“takas, takas”..

Question: Did you know where the shout comimg from?

Answer : From JO1 Galia sir, the assigned tower 5 guard during that incident sir.

Question: After that what happened next?

Answer : I instructed all duty jail officers at that time sir to immediately padlocked all
inmates in their cells.

Question: After that what happened next?

Answer : After seeing to it that all the main gates of both the Main Building and the
Drug Offenders’ Building were already being padlocked, I instructed again the
duty jail officers to be in their post.

Question: What transpired after that?

Answer : After that, I went outside to help pursue the escapee sir but I went astray, I
never had the chance to meet other personnel who help recover the said
escapee sir.

Question: After that what did you do next?

Answer : I went back to my post sir and I just heard afterwards that the said escapee
where already recovered with gunshot wound on his stomach, and was brought
immediately to Cebu City Memorial Medical Center (CCMC) hospital for
treatment thereat.

Question: After you heard that, what did you do next ?

Answer : After that sir, I just counted all the inmates and remain in my post after
knowing that only one inmate escaped.

withdraw in your statement?
Answer: None sir.
