1.0 Purpose
This procedure is to establish guideline for the installation of embedded plate on the second floor
slab of CCB – Switchgear Room prior to slab concreting works.
2.0 Scope
This procedure shall apply to all employees responsible for planning, supervising, and execution of all
activities covered by this procedure.

3.0 Roles and Responsibilities

3.1 VPCI Project Manager - shall be the overall responsible for the implementation of this
procedure ensuring that works are completed on schedule and in conformity with
drawings and specifications.

3.2 VPCI Project EHS Manager shall ensure safe implementation of this procedure promoting an
Incident and Injury Free environment.

3.3 VPCI & GTC Global Inc. QA/QC Engineer shall be responsible for the
implementation of construction specification and standards and see to it that this
written method statement of the work shall be carried out.

3.4 GTC Global Inc. Construction Manager – shall have the overall responsibility for the
direction, monitoring, coordination and control of all the construction activities
attached and related to concrete works and the overall Health and Safety procedure.

3.5 GTC Global Inc. HSE Engineer along with the Safety Officer shall ensure
implementation of all safety procedures, safety measures related to the nature of
works being carried out and in accordance with the HSE Plan.

3.6 GTC Global Site Supervisor shall be directly involved in the planning and execution
of the activities. He shall be responsible for the preparation of work plan, safety
action plan and safety.

.0 Procedure
Supervisors and workers performing welding, cutting, and/or bracing operations shall be trained
in hazard recognition, evaluation, and control; fire protection; and safe work practices; and shall
adhere to requirements in this procedure.

5.1 ELECTRICAL WORKS


5.1 Secure all necessary permit needed for the activity.
5.2 Conduct a pre-job briefing prior to start the work. Make sure that all working personnel had
their training for confined space, and ensure that they are capable to conduct rescue
procedures.
5.3 Set boundaries for the hard barricade. Install adequate signage before working.
5.4 The supervisor shall make sure that the preceding activities are approved prior to the
commencement of the next activity.
5.5 Layout & markings of electrical fixtures & devices indicated in the drawing plan provided by the
surveyor.
5.6 Provision of embedment prior to installation of electrical devices and fixtures.
5.7 Commencement of installation of electrical fixtures and devices and accessories.

.2 HOT WORKS

welding.

MATERIAL

5.2.1 Welding/Cutting Works


Whenever possible, cutting operations should be performed in areas that
are isolated and designated for such uses. Cutting must not be
performed in the following areas:

1. In the presence of explosive atmospheres.


2. In areas near the storage of quantities of exposed, ignitable
materials.
3. In sprinkled buildings while such protection is impaired.
4. In areas not authorized by management or by a hot work permit.

5.2.1.1 Site Preparation: Prior to beginning welding or cutting operations,


workers must:
1.0 Check the work area (35' radius) to ensure that no fire hazards
including oily or greasy materials are present;
2.0 Remove all combustible materials within 35’ not necessary for the
operation. Any combustible material that cannot be removed,
such as wood platforms, should be covered with a flame-resistant
material;
3.0 Check that all equipment is in good working condition;

5.2.1.2 Ventilation: Cutting or welding operations must be performed in areas with


adequate ventilation to keep fumes and gases within safe limits.

1. Local exhaust ventilation must be used when potentially hazardous materials


are being worked on. (Examples of potentially hazardous materials include,
but are not limited to, chromium, fluorides, zinc, beryllium, cadmium, lead, and
mercury).
2. Adequate exhaust ventilation must be used when using inert-gas welding,
plasma-arc cutting or carbon-arc cutting.

3. The metal surface shall be free of all chlorinated solvents during any welding or
cutting operations.

.3 INSTALLATION OF EMBEDDED PLATE


5.3.1 All elevation and coordinate values shall be indicated in the drawings or other
documents, unless otherwise stated.
5.3.2 The location of the embedded plates shall be in conformance of the approved
drawings.
5.3.3 G.I pipe (28.6 mm diameter x 2.3T) or angle bar shall be welded on top of the
embedded plates. Fillet weld stitches along the length of the plate to maintain
flatness and levelness of the plate. Please see attached drawings.
5.3.4 The pipe holder shall rest on the girder or beam reinforcement with anchor bolt
tied on the main bar to resist the movement of the plate during pouring.
5.3.5 During concrete pouring, Surveyor shall monitor and check levelness to ensure
elevation.
5.3.6 After the concrete pouring and once the concrete hardened and was cured, the
pipe holder shall be removed from the plate and the weld residue shall be
replenished with grinder and will be painted with primer and galvanizing plate
finishing.

6.0 Environment, Health and Safety

6.1 All personnel must wear appropriate PPE at all times. Personnel are empowered to stop work
if safety precautions are not in place.

6.2 The Job Site Supervisor shall ensure that all Safety requirements are put in practice, copy of
MS and JHA (Attached) are available on site for guidance and consultation.

6.3 Contractor Safety Representative shall carry out the safety site inspection to ensure the safety
requirements laid out in the safe work plan are followed during and after the completion of the
works.

6.4 Job Site Supervisor and Safety Representative shall be present at all time while works are in
progress.

6.5 Appropriate PTW to be prepared and implemented, modification without OWNER approval is
prohibited.

6.6 Assessed risk as necessary and communicated to concerned parties prior to start of any
work.

6.7 Safety briefing / Tool box talk must be conducted by Supervisors prior to start of work in the
language of workforce.

6.8 Incidents and injuries, damaged to property or near misses should be reported.

6.9 Good housekeeping to be maintained during the execution of activity.

6.10 A clear and good communication must be maintained by all personnel involved on the
activity.

6.11 Personnel must be trained/experienced and familiarized with the scope of works to be
undertaken. The location of marshalling point must be identified, emergency evacuation
procedure must be clear to all workers, exercises must be conducted.

6.12 Contractor Safety Manager / Representative shall review Safety Action Plan.
