SUBJECT : REQUEST FOR DATA ON THE RESULTS OF THE

QUARTERLY SUMMATIVE ASSESSMENTS IN ALL GRADE
LEVELS

DATE : July 30, 2021

In compliance with the request from the Office of Senator Sherwin Gatchalian,
coursed through the Office of the Undersecretary for Legislative Affairs, External
Partnerships and Project Management Service, this Office respectfully requests all
Regional Directors to provide and submit data on the results of the quarterly
summative assessments in all grade levels.
A template is provided in this Memorandum, which must be accomplished and
submitted on or before August 6, 2021 (Friday) through the Regional Operations
Secretariat (usec.regops@deped.gov.ph). Submissions should be consolidated at the
regional-level, verified, and signed by the Regional Director.
