OBSERVATION:
Chemicals were observed stored without
segregation, without labeling, and workers using
chemicals without wearing specific gloves nor
specific mask which harm the skin and lungs, waste
collected in random manner without identifying a
specific waste area and accumulated without
frequent disposing
RECOMENDATIONS include but not limited to:
-SBG has been instructed to correct the
observation and provide proper segregation, label
and display MSDS.
-SBG shall inspect all site workplaces and ensure
that any chemical which is stored at site in a safe
manner as per standard and rules requirements
according to relevant MSDS.
-SBG shall issue hazard communication program
including the chemical label system, Training to all
involved workers on the chemical hazards, MSDS
1 provision and publication into common project
languages to all site involved workers etc.
-SBG shall isolate any chemical storage area by
physical barriers with a proper ventilation and
storing temperature upon its MSDS and proper
signs and written instructions shall be displayed
and All required special PPE's according to MSDS
shall be provided while handling. Provide chemical
and proper mask is mandatory to who will work in
or deal with chemicals, Eye protection and long
sleeve is highly requested when handling chemicals
LOCATION: SVB / ZONE#14
OBSERVATION:
Lifting skip was being used without inspection sticker
by 3rd party, which could create risks of lifing failure
while suspension due to defected lifting accessories.
non-color coded slings inspected at other locations
(Piazza,SVB).At SVB Z#14 the area underneath
suspended skip has been observed not properly
barricaded. Spill kits not available beside liquid
chemicals. also noted at Piazza a blind spot lifting with
communication means available between operator on
the up-floor and rigger at the down-floor but language
barriers between both of them had been noted
RECOMENDATIONS include but not limited to:
-SBG shall remove of the observed skip for site usage till
to be inspected, calibrated and marked with inspection
sticker. - SBG shall inspect all site zones and remove any
un-inspected/ non-color-coded rigging accessories
from site. - SBG shall implement an effective inspection
system on all skips and all rigging accessories and
2 ensure that all rigging accessories in service have third
party certificates and safe without any defects.
-SBG shall train all site riggers on the safe work practice
including segregating any lifting zone and instruct
them to not use any lifting equipment without
inspection. - Lifting plan shall be edited for any critical
lifting process as classified in OHSA specially blind spot
lifting. - SBG shall consider the factor of language
barrier specially between crane operator and rigger
-Safety supervisors shall focus on lifting activities and
ensure that all lifting equipment being used are
inspected and marked with inspection sticker and
ensure that all other lifting control measures including
isolation and segregating the lifting zone are in place.
-SBG shall implement all relevant legal and
international standards requirements.

OBSERVATION:
Found an active work area within SVB / Zone#14 at site
without provision of adequate lighting while working
in inadequate light levels triggers the release of the
sleep hormone melanin which in turn reduces
alertness and increases the chances of errors and
accidents” which can cause personal injuries.
RECOMMENDATIONS:
Lighting is used for many different purposes to ensure
visual work can be done accurately, safely and in
comfort. The quality of lighting in a workplace can
have a significant effect on efficacy of workers. The
level of lighting that workers need varies depending
on the nature of the task, the sharpness of the workers'
eyesight, and the environment in which the work is
done. Good lighting in the workplace reduce risk of
occupational accidents and health problems and
promotes a better concentration, better visibility and
improve accuracy in work. Improved lighting can be
3 achieved by using more daylight, by changing the
position of light sources or workstation layouts and by
effectively using reflected lights etc. Adequate and
suitable lighting is essential for work in construction
site.
Contractor need to ensure that adequate lighting is
provided in each working area of the project to avoid
accidents at site.
Its supervisor responsibility to work in safe and
properly illuminated area and must not carry work at
dark areas as good lighting can decrease errors by
30-60 % as well as decrease eye-strain and the
headaches, nausea, and neck pain which often
accompany eyestrain.

OBSERVATION:
Observed webbing sling hasn't subjected to colour
code “damaged- Multi transverse cuts”. which indicates
to no periodic inspection has been conducted and no
pre - job inspection has been conducted by riggers,
case violates.
RECOMENDATIONS:
-Pre-job inspection shall be conducted prior to lifting
commencement to all lifting equipment and
accessories to ensure integrity of tools strength, pre job
inspection shall be evinced by a system form i.e. last
minute risk assessment checklist.
-Periodic inspection shall be conducted to all lifting
equipment and accessories, to ensure the tools
integrity.
- SBG shall remove and cut any defected webbing sling
to confirm not usage again by any misbehave or
human violation / mistake
-SBG shall train its own employees on the lifting
4 associated hazards, relevant safe work practice to
prevent such unsafe behaviors.
-SBG shall increase the safety supervision to cover all
the work zones at all times.
- SBG shall apply color coding attached with identified
webbing sling number, then preparing a list for their
distribution supportive with number and location for
easy inspection and tracking
-SBG shall implement all relevant legal and
international standards and HSE plan & DAR PMC
OHSEMS.

OBSERVATION:
Scaffolder has been observed with expired 3rd party
certificate and zone around it is not properly
barricaded and isolated which creates risk on the
operator and the pedestrians.
RECOMENDATIONS include but not limited to:
-SBG shall stop using the concerned scaffolder till
renewing the inspection certification.

OBSERVATION:
It has been observed that one harness was being worn
without inspection color code and one workers are
not complying with wearing the FBH all the working
time. Misusing had been noted everywhere as workers
tie-in the two lanyards of harness together or making
twisting between them or hocking in one anchor
point or at any point and sometimes at down head
point
RECOMENDATIONS:
-SBG shall ensure that all harnesses being used at site
are safe for use and any non - inspected or defective
harness shall be removed from job site. - SBG shall
implement an effective inspection system prevent any
usage of non-inspected harnesses. - SBG shall instruct
the site manpower not wearing any non-color coded
harnesses. - SBG shall ensure that all workplaces
covered by safety supervisors at all work shifts
enforcing the workers wearing their PPE's and for
6 correctly usage. - SBG shall instruct the construction
site supervisors to ensure that all the safety harnesses
being used by their workers are inspected and color
coded. - SBG shall take all measures to implement an
effective inspection and color code system to all safety
harnesses. - SBG shall ensure that all workers working at
height received work at height training and is marked
by work at height trained LOGO on helmet prior to start
any work at height activities. - SBG shall take
disciplinary action against any worker work at height
without tie his harness off.

Floors / Piazza. (OBSERVATION):
Some workers have been observed without
wearing basic PPE's (safety helmet) at workplace
this expose them to risk from nearby working
activities, Additionally some PPE's were observed
stored at floor in some locations inside the fire
hose cabinets and one harness lanyard was
observed not inspected. noted a shortage in
usage gloves specially for who working with hand
tools, and shortage in goggles specially who is
surrounding with ejection hazards or near to
expose to eye injuries, using surgery mask instead
of chemical or dust mask, working with chemicals
by short sleeve clothes
RECOMENDATIONS include but not limited to:
-SBG has been instructed to correct the
observation to provide the suitable PPE's in
compliance with the task needed, and take action
against the violators. - Refresher training on the
7 importance of wearing basic PPE's shall be given
to all site manpower by SBG. - SBG shall instruct
the construction supervisors to take care about
such unsafe behaviors and enforce their work
crews to wear their PPE's. - SBG shall increase the
number of safety supervisors to cover all zones at
all shifts and rectify any unsafe behaviors from
site workers. - SBG shall instruct the site workers
about the safe and correct way of storing the
PPE's. - SBG shall remove any PPE's stored at fire
hose cabinets and prevent storing anything inside
such serious equipment to be ready for use in
case of emergencies. - SBG shall remove any
harness not inspected and ensure that all
harnesses being used at site are safe and

OBSERVATION:
Some live electrical panels have been observed with
red inspection sticker with notes.
The main cable supplying the power to the
observed panel is not properly secured,
RECOMENDATIONS include but not limited to:
-SBG shall use the right agreed inspection system
(safe or not safe) tag/sticker and any unsafe panel
shall be removed from work site or disconnected.
-SBG shall re-inspect all site electric panels and
ensure that the panels marked with safe sticker
without notes is only used.
-SBG shall implement an effective inspection system
conducted on a daily basis to ensure that all site
panels are safe and secured.
-SBG shall increase the safety supervision to always
cover all the work zones.
-SBG should implement all relevant legal and
international standards and HSE plan & DAR PMC

-SBG should ensure that any live electric panel is
safe to use and contains all safety devices, e.g. GFCI,
earthing, breakers, screens, firmly locked, marked
with the electricians contact numbers, display
written instructions prevent conduct any electrical
maintenance or repair by workers.
-SBG shall secure all electric cables supplying the
power to panels against any damage could take
place.

OBSERVATION:
Shortage in the visual signs and escape routes arrows
guiding and leading to the nearest assembly points
and missing layouts showing the escape escape
routes and assembly point locations. Fire wardens/
marshal is not identified.
RECOMENDATIONS:
-SBG shall rectify the observation and provide
adequate escape routes to clearly lead to the nearest
assembly point.
-SBG shall provide the tunnel with layouts showing
the escape routes locations and paths and the
assembly points locations.
-SBG shall designate fire marshals at this area and
share and display their names and contact numbers.
-SBG shall train all involved site employees on the
emergence procedure. to get everyone fully aware
about his roles and responsibilities in case of
emergency and to get them aware about their
9 assembly point
-SBG shall conduct regular emergency drills to make
the involved employees familiar with the emergency
procedure.
-SBG shall train construction teams on the hazards
relevant to blocking work accesses to prevent any
unsafe behavior could cause any blockage to the
escape routes.

OBSERVATION:
Construction material is stored in random manner;
not barricaded and without aisles in between which
could cause trip hazards and serious incidents for
workers. with difficulty for handling and re-usage
RECOMENDATIONS:
-SBG should rearrange the material storage areas to
be more organized, barricaded, labeled and
provided with proper aisles for safe movement.
-SBG should train the riggers on the safe work
practice of material unloading and stacking/
arrangement to avoid mix stacking and to take into
account the erection sequence to prevent any
incident during erection due to erecting such mixed
stacked material.
-SBG shall train the site manpower on the hazards
relevant to material arrangement.
-SBG shall implement all relevant legal and
international standards requirements.

OBSERVATION:
Found a deep constructed opening for elevator not
covered being left open without adequate protection
measures which is located at Zone # 14. The existing
cover is found unsecured, has the potential risk of fall
off and has likely to cause death or serious physical
harms. REPEATED OBSERVATION!
RECOMENDATIONS:
Working near or in open inherits have potential
dangers of falls inside which may result in serious
accidents. Work zone protection shall always be
considered on priority basis.
SBG has to protect all openings having high risk of fall
inside. All openings must be protected with around
guarding system as defined in SBG HSE Plan (Chapter
# 11 - Floor and Wall Opening Procedure). OSHA 29
CFR 1910.23(a)(9) does require that every floor
hole into which persons cannot accidentally walk shall
be protected by a cover that leaves no openings more
than 1 inch wide and every opening must be guarded
by a standard railing, Toe boards and Cover. The
railing must be provided on all exposed sides, the
guard must consist of a hinged opening cover (of
standard strength and construction) and removable
standard railings on all exposed sides (again,
excepting that of the stairway entrance) and fixing
mandatory signs on all the sides of chamber for the
purpose of awareness.

RECOMENDATIONS:
-SBG shall take immediate action and secure the
observed box and well insulate the cables.
-SBG shall train the site electricians on the safe work
practice and the correct measures for protecting against
electricity.
-SBG shall inspect all other zones and ensure that all
exposed live cables are safe, secured, marked and
labelled
-SBG shall ensure that all safety measures related to
control the electric hazards are taken.
-Refresher training on electrical hazards and LOTO shall
be given to site manpower to increase their awareness
towards electrical hazards.
-Any work at zones contains mixed cables shall be
through strict work permit signed by the electrical
department after ensure the system being used by the
workers is not energized, and LOTO system is properly
applied if needed.
-SBG shall ensure that any electric works are being
conducted by competent electricians.
-SBG shall comply with all applicable relevant standards
and legal requirements, HSE plan & DAR PMC OHSEMS.

material which was observed in the area
shown in the observation has been removed
and placed in specified area for Chemical
Material Storage. This has been informed to all
workers and their supervisors to use the
proper PPE while working with Chemical
Materials.

certificate by third party by decelaring it fit for
operation. Also this has been informed to the lifting
department to not use any bascket which is not
