 AFFIDAVIT OF LOW INCOME AND NO PROPERTY

I, JULIETA ISABEL GABRIELA D. LLABORE, of legal age (42 years

old), Filipino citizen, married to VENGIE MILITANATE LLABORE and is
presently residing at Barangay I, San Fernando City, La Union, after having
been duly sworn to in accordance with law, hereby depose and state that:

1. I am the mother of JASMINE ANNE D. LLABORE, thirteen (13) years

old and who is presently applying for a scholarship grant;

2. My present occupation is a housewife while my husband is a part-time

tricycle driver who earns around 1,000.00 to 3,000.00 a month only;

3. I and my husband do not own any real property;

4. We do not file income tax return with the Bureau of Internal Revenue
(BIR) for the reason that we are exempted to file the same; and

5. I am executing this affidavit to attest to the truth of the foregoing

narration of facts and for all legal intents and purpose it may serve.
