Sub: Ooline tnnsfer of CGHS Cards of serving employees otr trlosfer from one CGHS
Covered Cify to another CGHS Covered City - Reg.

With refe.ence to ihe above mentioned subject the undersigned is directed lo state that
this Ministry has been receiving rcpresentations for allowing online t'ansfer of same CGHS
cards ofserving employees of Central Covernment on transfe. from one CCHS City
to anothe'

CGHS City. The maiter has been rgviewed and it has now been decided, in supersession
of the
earlier orders in this regard, to allow online transfer ofsame CGHS cards ofserving employees
ofcentral Government on transfe. from olre CGHS City to another CCHS City as per ihc ditrils
given under:

i) Serving employees on transtir f.om one CGHS Ciry to another CGHS City shall submit
an application to the Additional Director ofCCHS City (forwaded by the Minisl')
/Departm€nt from where he /shi i5 being transferred) along wilh copy of the transfir /
relieving order for transler of CGHS Cads to another CGHS Cily wilh an u0dettaking
that he shaLl be residing in the new City irt a CGHS covered area

ii) Addl. Director, CCHS of Cily shall ransfcr the card online and shall issue an

acknowledgement slip to the scrving employee. CGHS Plastic cards shall be retained b)
the serving employee.

iii) Serving employec in the new CGHS City submits an application duly forwarded by
his /he. Ministry /Departm€nt/ Office, certi$ing that CCHS cont'ibution is being
deducted, to the Additional Directo. of new CGHS City for acceplance of the transit
CGHS Cards to the data base of new City and allocation ofa CGHS wellness Centre in
new CGHS City along \ /ith proof of residenc€ for residing in a CGHS covered area'
Additional Di.ector, CCHS in the new City shall accept the cands in new City after

iv) lt is also brough! to the attention oi all luinistries and Depa(ments thal they are
responsible for surrender ofCCHS cards issued to Serving Employees. when the) are
transferred ao a non-CcHS Covered City.

These orders shall supersede all the earlier instructions issued on the subject and will be in
effect irom the date of iis issue.
