    POST ACTIVITY REPORT

    My activity regarding FBS screening at Plaza Medica specifically at Dr.


    Floranie Paredes starts at 6:00 in the morning. The expected number of
    participating patients is around 20, but since Dr. Paredes only accepts 15
    patients in the morning and the rest is in the afternoon, only 15 patients
    participated in the said activity. I ordered foods and drinks for Dr.
    Paredes, her secretary, and staffs at Molino Grills for at least 5 persons.
    The purpose of the said activity is to further strengthen our relationship
    and rapport to our doctors thereby increasing prescription and use of
    Imerid and other Pharmatrix products. I therefore recommend that I
    should have this kind of activity in the said clinic at least 2 times in 3
    months time.
