ROOT CAUSE ANALYSIS REPORT

Dept. EMD Equipment TDBFP-1B LOP-A Shift A-Shift

Equipment Code Stop Time: 12.30 hrs, 18.08.2021

Frequency Date of RCA 27.08.2021 Start Time: 15:30 hrs, 18.03.2021


Name of Fault / Breakdown (Please state the parts damaged , Location if any )
TDBFP-1B booster pump LOP-A was in running condition.
TDBFP-1B Booster pump LOP-A tripped on overload protection
Condition Prior to failure ( Please state the past performance , history )
Unit-1 was in synchronized condition @load 300MW with TDBFP-1B booster pump LOP-A In service.

Observations
1. Noticed tulip contact to motor terminals on R phase found in damaged condition (Picture attached in Sheet1 for ready reference).
2. Changover happened between LOP's LOP-A tripped and LOP-B came to service.

Causes of failure/ Root Cause:


Why Why Analysis ?

1 .Why TDBFP-1B booster pump LOP-A tripped on over load protection?


Due to breakage of cable from OLR to tulip contact which caused negative sequence current which eventually operated OLR.
2. Why cable got broken?
The connecting cable got broken due to looseness in lug crimping.
Root Causes
1.Breakage of lead caused tripping of motor on over load protection
Action Taken
1. Since the damaged tulip contact set is beyond repairable replaced entire set of tulip contact.
2. Motor taken to line and healthy.

Recommendations to avoid recurrence

1. Inspection of tulip contacts need to be checked in Preventive maitenance works.
