Lima, August 15, 2011

WHEREAS Vicky Esther AGUIRRE CARBALLO filed a petition to be registered in the

National Register of Persons with Disabilities.
WHEREAS, to request her registration in the Individuals Registry of the National
Register of Persons with Disabilities, the petitioner, Vicky Esther AGUIRRE
CARBALLO, filed documents acknowledging her condition as a person with disability
under the following Diagnosis: Ataxic gait (R26.0), Other abnormalities of gait
and mobility and unspecified abnormalities (R26.8), Other specified paralytic
syndromes (G83.8) as per Certificate of Disability 33, dated June 7, 2011 issued by
HOSPITAL II LUIS HEYSEN INCHAUSTEGUI ESSALUD – LAMBAYEQUE.
Furthermore, the petitioner submitted a sworn statement and complete
documentation in accordance with the regulations in force.
WHEREAS paragraph a) of section 12.1 of article 12 of Law N° 27050, the General
Law on Persons with Disabilities sets forth that, the registration in the National
Register of Persons with Disabilities, under the control of the National Council for the
Integration of Persons with Disabilities - CONADIS, is free and shall include the
filiation of the persons with disability and their family members, among others.
WHEREAS article 17 of Supreme Decree N° 003-2000-PROMUDEH, amended by
Supreme Decree N° 003-2006-MIMDES, establishes the requirements and procedure
for the registration in the Registry.

WHEREAS, by means of Note N° 809-2008-MIMDES/OGAJ dated November 11,
2008, the General Counsel of the Office of the General Counsel of the Ministry of
Women and Social Development - MIMDES, ratifies Report N° 1439-2008-
MIMDES/OGAJ dated July 9, 2008, sent to CONADIS according to Note N° 112-2008-
MIMDES/DVMM dated July 10, 2008, and signed by the Vice-Ministerial Office for

Women advises the Presidency of CONADIS, as CONADIS’ budget administrator and

legal representative, to take on the Executive Secretariat until the Regulations on
Organization and Functions of CONADIS is enforced.
WHEREAS, in this regard, it is appropriate to include Vicky Esther AGUIRRE
CARBALLO in the Individuals Registry of the National Register of Persons with
Disabilities.
In accordance with the provisions of paragraph a) of section 12.1 of Law N° 27050,
the General Law on Persons with Disabilities, and section 13.9 of article 13 of
Supreme Decree N° 003-2000-PROMUDEH, amended by Supreme Decree N° 003-
2006-MIMDES,
Be it therefore RESOLVED:
Sole Article: To include Vicky Esther AGUIRRE CARBALLO in the Individuals
Registry of the National Register of Persons with Disabilities under the control of the
National Council for the Integration of the Persons with Disabilities – CONADIS.
