1 Forward filled-out ATM application form to Ms. Jhade Ramos Every Wednesday with attachment of the ff:
a. Photocopy of Valid ID with 3 specimen signature

1 Forward Filled-out Government Application form (SSS,PHIC,BIR,HDMF) to Ms. Zhela Every Wednesday
a. HDMF (Pag-Ibig)

b. Philhealth (PHIC)

c. BIR
c.1 BIR Form 1901
Contract- Workers Rate Form
1 Send the worker's Rate form thru the designated MS Teams Channel (Recruitment-Compensation)
2 Hr Compensation will check and validate the WRF (Worker's Rate Form)
3 Once validated, recruitment will process the worker's contract. (Draft Contract Only).
