    1) An inspection of the GAS SUASA found that the mass of the MOB lifebuoys was 2.5 kgs instead of the required 4 kgs according to regulations. 2) The root cause was determined to be that the 4 kg lifebuoys had been interchanged during maintenance and painting. 3) Corrective actions included checking and weighing all lifebuoys, finding the interchanged 4 kg lifebuoys and placing them properly on the port and starboard sides of the Quick Release Arrangement. Preventive actions included marking lifebuoys with their proper weight and conducting regular inspections.

    (Temuan)
    According to LSA Code Chapter 2.1.1.7, the Lifebuoy intended to operate the quick release
    arrangement (MOB lifebuoys) should have a mass not less than 4 kgs. However the MOB
    5.31 lifebuoys onboard was found have a mass of 2.5 kgs.

    Menurut LSA Code bab 2.1.1.7, Lifebuoy diperuntukkan untuk dioperasikan Quick Release
    Arrangement (MOB Lifebuoys) harus memiliki berat tidak kurang dari 4 kgs. Akan tetapi MOB
    Lifebuoy diatas kapal ditemukan memiliki berat 2.5 kgs.

    Root Cause Lifebuoy 4 kgs interchanged to other place during maintenance & painting.

    Lifebuoy 4 kgs tertukar ke lain tempat saat perawatan & pengecatan.


    Corrective Action Checked and weighing all lifebuoy, founded lifebuoy weight 4 kgs interchanged to
    (Tindakan Perbaikan) other place. Lifebuoy weight 4 kgs marking and placed on the Quick Release
    Arrangement (MOB) port & starboard side.

    Mengecek dan menimbang semua lifebuoy, ditemukan lifebuoy degan berat 4 kgs
    tertukar ke lain tempat. Lifebuoy dengan berat 4 kgs diberikan tanda dan ditempatkan
    di Quick Release Arrangement (MOB) sisi kiri dan kanan.
    Preventive Action Marking Lifebuoy with weight 4 kgs and inspect regularly.
    (Tindakan Pencegahan)

    Memberikan tanda Lifebuoy dengan berat 4 kgs dan melakukan pemeriksaan rutin.
    Close-Out evidence might be; documents, procedures, photos, certificate, service report, delivery order, etc.)
