(Temuan)
A sample charts used for voyage No. 3729 (Tg. Priok To Cirebon) was found not fully
corrected. A review of the cumulative list of Indonesian Notice To Mariners indicated
4.12 that two consecutive correction had not been applied during month September 2021.

Contoh peta yang digunakan untuk pelayaran no. 3729 (Tg. Priok ke Cirebon)
ditemukan tidak dikoreksi. Tinjauan List Kumulatif dari NTM menunjukkan 2 urutan
koreksi tidak di koreksi selama bulan September 2021.

Root Cause The new chart just received on board and NTM just received in 2 days.
(Akar Masalah) Miss 1 chart to correction. Inspector need to fully corrected the chart.

Peta baru, baru saja diterima diatas kapal dan NTM baru diterima dalam 2 hari.
1 peta terlewat untuk dikoreksi. Inspector mengharuskan peta terkoreksi.

Corrective Action Chart No. 3729 has been completed up-date correction NTM weekly 38/21.
(Tindakan Perbaikan)

Peta no. 3729 telah selesai diperbaharui untuk koreksi NTM mingguan 38/21.

Preventive Action Regularly update Notice To Marine chart & publication correction every weekly.
(Tindakan Pencegahan) Monitoring of voyage chart corrections by the Master.

For Improvement Action to avoid this deficiency in the future, Master gave a training
for all officer on board as follow:

1) Chart correction techniques

2) Checking and updating of chart corrections from Cumulative Notices to
Mariners (which published every 6 months) and Weekly NTM

Perbaharui secara teratur NTM untuk koreksi peta & publikasi setiap minggu.
Memantau koreksi peta pelayaran oleh Nakhoda.

Untuk Aksi Perbaikan guna menghindari temuan ini dimasa yang akan datang,
Nakhoda memberikan beberapa pelatihan kepada seluruh perwira diatas kapal
sebagai berikut:

1) Tehnik mengkoreksi peta

2) Memeriksa dan memperbaharui koreksi peta dari Kumpulan NTM (yang
diterbitkan setiap 6 bulan) dan NTM mingguan
