    The inspection/audit closed-out form documents an inspection of the MT. GAS CAMELOT conducted on October 30, 2021 in Pulau Sambu. The inspection found that the fire plan endorsed by the Administration/Flag State was not available onboard. The root cause was that it was in progress to arrange endorsement of the Fire Plan. The corrective action was to submit the Fire Plan to the administration for approval. The preventive action was that the Master will make sure all drawings are approved by the Administration.
