Welcome to the Electrical Installation and Maintenance Grade 9 Self-Learning


Module (SLM) on Requesting Tools, Equipment and Materials!

This module was collaboratively designed, developed, and reviewed by educators


both from public and private institutions to assist you, the teacher, or facilitator in
helping the learners meet the standards set by the K to 12 Curriculum while
overcoming their personal, social, and economic constraints in schooling.

Lesson Requesting Tools,
1 Equipment, and
Materials
Request forms are very important. Construction workers need these to
request supplies and materials as they start to work and as they return after
using what have been requested. Requesting tools, equipment, and materials
need to undergo a correct process so that everything will be in order.

What’s In

In this lesson, you will learn the principles in the preparation of


electric and hydraulic tools. This module will guide you to select the
needed tools, equipment, and materials per job requirement; prepare a
list and request them according to the list prepared. Requests are done
based on the company standard operating procedures (SOP). So,
whenever there are materials, tools, and equipment unavailable, these
are immediately substituted and provided for so as not to sacrifice cost
and quality of work.

Remind your learners that this part of their lesson is just a
refresher of the previous lesson on filling out of forms. Remind
them to be accurate when they fill out forms. They need to write
legibly and avoid erasures.

What is It

Filling out forms is not new to you, isn’t it? You do it in many instances in
school. In answering questions, taking an examination, or upon enrolling, you have
to fill out documents. Filling out forms is also a necessity in EIM. Requisition and
borrowers form is part of the system.
Below are sample forms. Study them and take note of the content.

The requisition slip form is a form to fill out when a learner makes an order
request to accomplish a project or task.
