EVALUATION INSTRUMENT
Level 1

This level one evaluation will relate to how the participants feel at the end of the technology
coaching session. This will be a Google Form that participants will fill out before leaving the
session. The ratings will go from strongly agree, agree, neutral, disagree, strongly disagree.

Level 2

Before the coaching session an online questionnaire will be sent out to participants with the
topics listed. Participants will rate how knowledgeable they are on each topic with the ratings
from 1-5. This pre-assessment will allow coaches to modify instruction if needed. After the
coaching session, participants will fill out a 3-2-1 survey. Participants will write down three
things they learned, two things they still want to learn, and one questions they still have. Also,
participants will answer three questions about what they liked most, least, and what they are
interested in for future sessions.

Level 3

For the level three questions, I feel an interview with the participants and administration would
be most beneficial to see how the coaching sessions are relating to their teaching and how it is
changing the dynamic of the school. These interviews will allow for change to upcoming training
sessions or provide additional support if needed.

Teacher:

1. What resources have been provided for you to continue using the training sessions in
your classroom?
2. Did you have any problems? If so, were they addressed quickly and efficiently?
3. What additional resources or training do you need to continue using the strategies learned
in your classroom?
4. How has your students responded to the new strategies being implemented?

Administration:

1. Have you noticed a change in ________’s teaching and/or students?


2. How have you supported ____________?
3. What additional support do you need to support your teachers?

Level 4

To fully see if the professional learning is being applied in the classroom, observations is the
way to go. Participants should schedule a time with the trainer within a month of the professional
learning session. TKES is a familiar teacher rating scale that is used with observations.

Although, this will not be implemented in the TKES framework it will have the same grading
scale. Each level is as follows:

Level 5

Level five of Guskey’s level of professional development is all about student outcome. In order
to see the outcome of students you would have to have conversations with the teacher or
administrator to gather data. Some forms of data to ask teachers for; classroom assessments,

standardized tests, student questionnaires, and end of unit assessments. I would ask teachers
to gather the data they have for their class and be ready to discuss the following questions.

2. What impact do you think the professional learning had on students?

Administrator:

1. What do you notice about __________’s data compared to other teachers who did not
attend the professional learning?
2. What impact do you think the professional learning had on the teacher?
