I am Aquino Jonathan of (Section) 12StemB11 with failing grade/s in the subject/s Work Immersion
Imm002 in the 2nd semester, SY 2020-2021. I am participating in the Remedial Class with the full
understanding that:
 Participating in the Remedial class does not automatically mean a passing grade.
 The highest possible grade from the Remedial class is 80.
 I can attend the Remedial class only after I have paid my dues and presented a photocopy of the
official receipt to my Remedial teacher.
 100% attendance to the Remedial class is required.
