MECHANICS FOR THE ONLINE PRACTICAL EXAMINATION ON HISTORY TAKING

1. This is a synchronous graded activity.


2. Each student will be assigned a case.
3. The facilitator will act as the patient. The student will get the medical history.
4. The interview will be done INDIVIDUALLY (one-on-one).
5. Each student will be given 12 minutes to conduct a complete history.
6. You will be required to turn on your camera during the entire conduct of the
history taking.
7. Please be in your uniforms (type A or B) or a white collared shirt if you do not
have a uniform.
8. You will be asked to verify your identity before starting the exam. Please have
your school ID, or any valid ID ready.
9. The facilitator will briefly inspect your surroundings. There should be no other
person in the room aside from you. There should be no notes/screen/visual
aid on the wall or notes/gadgets anywhere within your reach.
10. The facilitator will ask you to share your desktop. No other application or
browser should be open other than Zoom.
11. You may have a blank sheet of paper where you can write your notes. Your
facilitator will inspect if the paper is indeed blank.
12. You may be asked to stay in the breakout room in Zoom while waiting for your
turn.
13. The practical exam will cover history taking only (from general data to personal
and social history).
14. The session will be recorded.
15. You are NOT allowed to communicate with your classmates in any form during
the entire duration of the practical exam, until the entire class is done with the
activity.
16. You are not allowed to disclose any information about what transpired during the
exam.
17. Should you or your facilitator encounter technical problems that will affect the
seamless conduct of the exam, it may be re-scheduled at a date and time
agreeable to both parties.
