couple of months when a foster placement is found or when he can return to parents’ care.

Y.C.J.A. INVOLVEMENT: Police were involved during an indent in the home where Evan ran
away however, no charges were made. He has had past experience with criminal related
behaviour however never received charges from police due to being under the age of 12 and the
severity of the crimes.

C.Y.F.S.A. INVOLVEMENT: Children's Aid Society (CAS) became involved after video
footage from within the home was made public. Evan and his younger sister were both removed
from their home and parents’ care. CAS took over with the aim of reunification after all parties
(both children and parents) have received treatment. CAS has placed Evan in our care until a
foster placement has been found and/or it is deemed acceptable for him to return to his parents’
care. 

RIGHTS REVIEW DATE: A right review date was completed the day he was brought into the
program, October, 13th, 2021. Another rights review date will be 3 months from the last on
December 14th, 2021.

 Evan had multiple detentions within a school year for small rebellions (ie. chewing gum) 
 Was suspended from school on multiple occasions. 
 He was suspended twice in less then a month (October 6th, 2020 and October 22nd,
2020)

 “Evan was physically aggressive towards a classmate, gouged his fingernails into the

SOCIAL HISTORY 

REASON FOR REFERRAL: Under the care of Children's Aid Society after being removed
from the home in September 2021 due to violence and abuse in the home. Referred to this
agency for temporary placement until a home setting placement can be provided for this
individual. 

PREVIOUS ASSESSMENTS: 

Was assessed within the school system which concluded he has above average intelligence in
2017.

Was classified under the category of Conduct disorder in 2014, however no formal diagnosis was
made.

At the age of 4 Evan was classified as conduct disordered.

Mental health history: At the age of 4 Evan was classified as conduct disordered. He is
currently working with a psychiatrist as of fall 2021, 

FAMILY SITUATION: Evan grew up in a home with his younger sister, mother, and
stepfather for most of his life. His parents divorced in 2014, from multiple incidences of
domestic violence. Michael received full custody of both Evan and his sister and for 7 months
raised them on his own. Karin began living with them again however custody did not change. In
2021 both Evan and his sister were removed from the home due to verbal, physical, and
emotional abuse as well as neglect toward Evan from both his mother and stepfather.

INTAKE INTERVIEW AND FAMILY/COMMUNITY RESOURCES PROVIDED 

Intake interview was completed on October 10th, 2021.

Evan was able to identify what he needs from this program. 

He was able to recognize what he may struggle with during his stay such as not seeing his
family, being in a social setting with peers constantly and not having space and time to himself.

FAMILY INVOLVEMENT /STATUS/CONTACTS:

Both Karin his mother and Micheal his stepfather are involved in his life right now, they have
asked to be as involved in Evans care process as possible. His parents are actively working with
the agency and outside support to build parenting skills and repair family relationships. However
neither have custody and Evan is currently under temporary care within this agency. However
the ultimate goal is for Evan to be permanently placed in a home, preferably back in the care of
his mother and stepfather.

CLIENT AND/OR GUARDIAN’S ASSESSMENT OF NEEDS:

SOCIAL RELATIONSHIPS: Strong relationship with my sister. Potentially fragile


relationship with stepfather. Inconsistent relationship with mother. Very few strong peer
relationships. Few positive relationships with adults within and outside of school. Family
relationship work such as parenting classes or family counselling would benefit Evan and his
family in healing past traumas, forming healthy relationships, and avoiding further challenges.
Group activities and recreational activities could benefit Evan by having a safe, monitored, and
structured environment to form healthy peer relationships. 

EDUCATION: Currently attending the 6th grade no accommodation or modification to


education are in place. He was assessed at school and they discovered he has above average
intelligence. Has missed several days of classes within the school year due multiple suspension,
he completed all credits up to the 6th grade. Evan has experienced bullying from peers at school.
Evan could benefit from having a support staff at school to work one-on-one with him to help
hold him accountable to staying on task and ensuring appropriate behaviour. 

EMPLOYMENT: Not employed, as he is under the legal working age.

RECREATION:  Evan is currently not enrolled in any extracurriculars or in any community


programing. Evan would benefit thoroughly from structured programs outside of the school and
home setting. This will help Evan keep a routine and keep him busy after school instead of being
home alone.  

SPIRITUAL: Currently no religious or spiritual beliefs/community to be mentioned. If he


expresses interest in exploring spiritual beliefs or religion we can connect him with resources to
do so. 

LEGAL ISSUES: Parents have gone through a divorce and mother no longer has custody;
Micheal has sole legal custody over both children. Currently children are temporarily in society
care; through a temporary care agreement, CAS has temporary care and custody of children. Has
a tendency to participate in illegal activity such as violence, theft, arson, drug and alcohol use.
However due to the insignificance of crimes and his age there have been no charges. Prevention
intervention is key to prevent possible serious legal consequences due to his behaviour. Also,
education about the laws, and his rights are essential since Evan could be at risk for criminal
charges. 

SUBSTANCE USE/ABUSE: Prior use of cigarettes and cigars and consumption of alcohol
(underage drinking). Prevention intervention is key to prevent and understand possible
developmental and legal consequences due to his behaviour.

LIFE SKILLS: Current life skills: cooking, cleaning, laundry skills, recreational sports skills,
independence, Educationally inclined, able to express his needs to others. 

Life skills to be improved upon: self-regulation, interpersonal relationship skills, empathy,


coping techniques and skills.

GOALS 

Short term goals

Goal 1: Getting Evan involved in one recreational activity outside of school or home as an outlet.
This goal will be completed by signing up for one activity he enjoys such as a sports team. Evan
will be signed up for an activity within the week of October, 18th - 22nd and we will know this
goal has been completed when he has attended his first meeting of practice by November 18th,
before the next goal meeting. 

Goal 2: Improve self regulation skills specifically regarding physically violent outburst towards
peers. This goal will be completed within the time frame of now and the next goal review date
(October 18st, 2021 - November 18st, 2021). We will know this goal has been completed when
there has been no suspensions within the month and less phone calls and reports from teachers
regarding physical violence insendence with peers compared to other months this meaning less
than 5 incidents. This will be accomplished by working alongside Evan through self regulation
interventions. 

Goal 3: Within the first month of Evan living within the group home he will feel more
comfortable with his new living environment. This goal will be accomplished by implementing a
survey the first few days of Evans stay as well as one day before the next goal meeting to assess
how he is adjusting. The survey will be given orally and informally by using a ranking system of
1-5 on each question, each question will be about how comfortable he is with daily tasks, his
peers, staff and whether or not he is enjoying his time here so far. If the ranking improves by the
next survey we will know the goal has been completed. 
Long term goal

Goal 4: Ongoing goal for this family as a whole is to rebuild a positive family dynamic. We will
know this goal has been completed when Evan and his sister, mother and stepfather have
completed their portion of treatment and intervention and CAS has placed Evan and his sister
back in the home and deemed it a safe environment. This goal should be completed within a year
from now that being around October 2022. 

STRATEGIES FOR INTERVENTION (THIS IS ALSO CALLED “MEANS OF


ACHIEVING THE GOALS” IN SOME REPORTS) 

Goal 1:  To help Evan get involved in one recreational activity outside of school or home as an
outlet, we will find out what activities Evan is interested in participating in. This could include
doing some research about activities in his area, or even taking a group to the park to play a
couple different sports games, building activities, drama games and others to figure out what
types of games Evan enjoys therefore make the diction of what club or activity to sign him up for
much easier. Secondly we will look at what clubs and activities there are in the area and then
help Evan sign up to the one he's most interested in. 

Goal 2: Helping Evan achieve his goal of improving self-regulation skills in terms of physical
violence with others. This can be done by incorporating the intervention called zones of
regulations, a system used to help youth understand how they are feeling and what techniques
they can use to help regulate their emotions and avoid negative behavior. For example, the zones
will be colored, and each represents a different emotional state; in each zone, there are activities
and strategies to manage the behavior and move to a calmer zone. This intervention allows a
chance for Evan to practice recognizing his emotional state and implementing strategies to
manage his behaviors and reactions. These strategies will be determined by talking to evan about
what he thinks would be helpful in those moments as well as input from staff and evidence based
research. Another intervention that can be utilized is helping Evan expand his emotional
vocabulary. This can be done one on one with a support worker by debriefing after Evan's
behavioral reactions to explore what kind of emotions he was feeling. This allows Evan to learn
about different emotions, what they feel or look like so he can better communicate to others how
he is feeling and be supported by staff instead of reacting with explosive behaviors. The same
effect can be accomplished by explaining scenarios of other individuals experiencing a variety of
emotions and having Evan identify what kind of emotions they are feeling.  

Goal 3: To help Evan adjust properly to a new home environment we will first let Evan decorate
his assigned room to ensure he has a safe space that is personalized to him. This will help him
become more comfortable within a new space as he can bring things from his family home to
integrate into his room. Secondly, we can slowly introduce Evan to the routine of the home by
partnering him with a senior resident who has contributed to the home for 6 months or more.
This senior resident can build a relationship with Evan, show him around the home, and partner
with chores. This will help ensure a slow and warm welcoming into the residence. 

Goal 4 : To help Evan in building a positive family dynamic and family relationships while in
care, we would recommend and be willing to facilitate visitations/home visits for Evan with his
parents. This would also help ensure that while they are separated and he is in care, that he is
aware of his parents’ presence/support, continued desire to be and remain active in his life, and a
reminder that they too are working on bettering themselves for him. Secondly, we would
introduce family/group counselling so they can better their family dynamic as a whole by
addressing the issues and needs of the family on top of individual needs (done through individual
counselling).

NEXT GOALS REVIEW DATES WITH CLIENT:

Goals will be reviewed and reassessed monthly, so the next goals review date will be one month
from now (November 18, 2021).  The goals created above are tailored to be completed within a
month's time frame before the next set of goals are established and the success of the previous
goals are addressed.

SUMMARY

Evan’s stay in this residence is temporary as the ultimate goal is to find this individual a permit
placement within the foster system. However, preferably placed back with his mother and
stepfather in their family home in the event CAS approves that placement after each family
member has gotten support or treatment and is prepared to be reunited. 

Evan will work on the goals that we have implemented now, then each month we will reflect and
create another goal that is suitable with his personal resiliency. The next plan of care date will be
in two months from now set for December 18th, 2021. After Evan has had time to settle into the
agency and get time to implement some of the strategies and see whether the interventions in
place are helpful for Evan’s development of skills. Also this plan of care needs to be reviewed
soon due to needing to reevaluate Evans living situation by connecting CAS for status on living
placement. 

The prognosis is that with consistent care and treatment of all parties reunification is a highly
attainable outcome. By working towards our monthly goals Evan’s risk factors will be reduced
or eliminated to build up resiliency in the form of self regulation and conflict management skills,
gaining a sense of purpose and worth, and fostering healthier family and peer communication
and support.
