Area of Concern
a. The current rate of progress of the post-hydro testing activities of
flushing and reinstatement are too slow to achieve M42 Project
Mechanical Completion in Process units.
b. The number of final walk arounds to complete is an increasing
challenge over M40 to M42. Punch A killing will be a priority and the
Punch B killing on process area utility systems is likely to become an
impediment to System RFSU of utility systems in Process Areas.
c. The resource level of electrical, instrumentation and controls
inspection personnel is inadequate to support the loop checks,
function checks, reinstatements, and final walk arounds.
d. Quality of work in the Hydrogen Manufacturing Unit is unsatisfactory
and further rework is identified. Improved coordination of work
sequence and progressive inspections is required.
e. SSP04 remediation, reconditioning of panels and panel replacement

Action to Area of Concern
a. JGCS has added direct labour and supervision to perform post-hydro
test activities. Letter issued to JGCS to seek an explanation of the
recovery plan for above ground piping and readiness for final walk
arounds in the Process systems.
b. IPMT are focused on the final walk arounds required for Non-Process
Systems and in Utility Areas, as few Process system walk arounds are
being implemented. IPMT and OPT resource review to be progressed
in early November to ensure there are adequate responsible persons
identified for walk arounds within every Utility and Process unit.
c. IPMT resources are being supplemented by OPT resources. IPMT and
OPT are seeking to identify suitable people to provide support.
d. JGC has added technical supervision resources and direct labour. is
coordinating actions with GS. Further action will follow if collaboration is
ineffective.
