Authorise permit. The Master is fully responsible for the


authorization of enclosed spaces entry .Chief Engineer,
Chief Officer and the Second Engineer may be additional
Authorizing Officers in case the Master is unavailable to
sign the entry permit.
Relevant Risk Assessment carried out.
Make sure the Enclosed space atmosphere will be
monitored while work is in progress. Constant ventilation
of enclosed space must be.
B. HOT WORK
1.
Hot work in port can be considered, permission must be
obtained from the terminal if alongside, and/or port
authorities as required by local regulations.
It will be necessary to obtain prior consent and
confirmation from Fleet Management Team when hot
work must be carried outside the designated space

Area cleaned, safe, isolated, gas tested, ventilated down to
not more than 1% LFL, free of toxic gases and all
flammable substances.

The safe distance of Fuel oil tanks from the Hot Work

C. WORKING ALOFT
1.
Conduct Tool box meeting. Planned work should be
discussed and approved by the SMT, including making an
assessment

Equipment.
A safety harness with lifeline or another arresting device
must be continuously worn when working aloft or over
side. A safety net should be rigged where necessary and
appropriate. Additionally, where work is done over side,
buoyancy garments must be worn and a lifebuoy with
sufficient line attached should be kept ready for immediate
use

D. FIRE IN PUMPROOM
1.
Sounded the Fire alarm. Inform OOW. Go to Emergency
stations. Account all personnel Prepare stretcher and
resuscitator. Position of the vessel fixed on the chart.

While in port- to stop all Cargo, Bunkering, Shut
manifold, Inform shore personnel.

3. Stop all ventilation. To close all ships Fire flaps, dampers.

4. Electrically isolated Fire


5. All Fire-fight equipment in ready , Fire man ready and
equipped with protecting clothing and BA sets.
Determine the size and type of fire. Commenced Fire-
Fighting locally using appropriate fire extinguishing agent.

Constantly maintain cooling to prevent the fire spreading
to adjacent spaces. To check for Toxic gases .
If necessary, request the assistance. To inform DPA,
secure the VDR data. If fire out of control abandon ship.
E. DAMAGE CONTROL DUE EXCESSIVE LIST
1. Sounded the alarm. Crew mustered to Muster Station.
Position of the vessel fixed on the chart

To determine cause of list. Assess the effect of list on
Hull/Machinery Pumps Pipes.

To check for Oil Spillage.

Sound all Tanks and Bilges carried out.
5.
Check if cargo can be transferred to another tank to bring
ship upright. Consider ballasting or de-ballasting to reduce
the level of list. To complete information for Damage
stability assistance.
6.
To determine assistance needed Tug/Salvage/Rescue.
Inform Company, P&I and investigate Company
Contingency Plan.
Advise Port State Control for assistance. If necessary, to
use GMDSS equipment to send a distress message or
Abandon Ship.
Secure VDR data
F. FLOOD OF ENGINE ROOM
1.
Sounded the alarm. Crew mustered to Muster Station.
Position of the vessel fixed on the chart.
2. To close water tight doors. Identify source of leakage .
Isolated area and to protect electrical equipment .

3.
Start to remove the flooding affect by emergency
discharge pumping out the affected space.
Maintaining the ship in a stable position.
4.
Start ballasting or de-ballasting operation to reduce the
level of flooding. If need , change of course and speed to
reduce level of water ingress.
5.
Determine possible final trim, list and stability due to
flooding.
6.
Inform Company, P&I and investigate Company
Contingency Plan.
Contact Emergency Response Centre for assistance.
Advise DPA. Advise Port State Control for assistance. If
necessary, to use GMDSS equipment to send a distress
message or Abandon Ship.
