Threats:
1. Damaged cables & accessories, protection cct. not
functioning, fuel spill on machine, fire due to
overheat of equipment.
2. Injury due to equipment movements along work
group.
3. Dust emission, ergonomic of personnel.
4. Fire, explosion and production plant tripping due
to UV ray detector actuation.
5. Temperature extremes due to open area.
6. Personal injury due to ‘fall into’ deep trench.
Controls:
1. Daily check accessories condition, replace if
necessary, check protection devices at regular
intervals. Avoid fuel spill, clean up spill and
dispose of. Provide FE with equipment. Use
correct rated equipment for work.
2. Drive, move equipment carefully along ROW to
avoid accidents.
3. Spray water to suppress dust.
4. Follow PTW procedures.
5. Provide cool water to personnel. Do not allow
personnel to rest underneath the machinery.
6. Do not work close to trench edges. Ensure trench
edge surface is intact, without any cracks. Avoid
walking close to trench edge.
Emergency Response:
1. First aid to be rendered to the injured,
immediately
2. Inform PM/ HSE/ CM/ Admin for necessary
supports.
3. Familiarise the team with fire, medical &
evacuation emergency response procedure.
