DECLARATION

I, Anuradha Bajpai spouse of Sanjay Bajpai aged 46, an Indian Inhabitant presently
residing at 6 Upendra Nagar, Durgakund, Varanasi do hereby solemnly affirm and declare as
under:
1. That Sanjay Bajpai and Hari Shankar Bajpai, the deceased, was holding 8000 shares
in F.D.C. Limited covered under Folio No. 0003138 and Share Certificate No(s)
00043163, bearing Distinctive Nos. 0136293001 to 136301000.
2. That (Sanjay Bajpai), the deceased expired on 28-07-2017.
3. That I am a legal heir of the said deceased. I am the (wife of and daughter in law of
Hari Shankar Bajpai, 18-10-2015).
4. I do desire to make claims of title of the said securities held by the Sanjay Bajpai and
Hari Shankar Bajpai. I hereby agree to renounce all my rights existing as well as they
may accrue to me in future in respect of the aforesaid securities.
5. I declare that I have no objection whatsoever in transmitting the said securities in the
name of the Anuradha Bajpai.
I am executing this declaration to be submitted to the concerned authorities of the
Company.
I hereby state that whatever is stated herein above are true to the best of my
knowledge.
