Re: Mr. Yuxiang Meng

Thank you for seeing Mr. Meng, a 21 year old overseas student chef from China who presented with symptoms suggestive of Commented [OET1]: 21-year-old
acute mania.

Mr. Meng initially presented on 2/03/11 with complaint of upper respiratory symptoms for five days. On examination, he
had mild pharyngitis and rhinorrhea. His mood was elevated and he was experiencing delusional thoughts. A provisional
diagnosis of mania and mild viral illness was made. He was referred to local community mental health and Diazepam 10 mg Commented [OET2]: diazepam
four times a day was prescribed.

Mental health team corroborated the provisional diagnosis and made the assumption that Mr. Meng was in no immediate Commented [OET3]: The mental
danger to self or others. Furthermore, he was keen to avoid hospitalization due to social stigma and wanted to continue
treatment with his general physician due to language issues. His labs showed hypokalemia and mildly decreased protein
levels. He was commenced on quetiapine 50 mg twice a day, potassium tablets, R/V in 3/7 and dosage of Diazepam 10 mg Commented [OET4]: diazepam
was decreased to either twice or thrice a day.

Today, Mr. Meng presented with elevated mood, pressured speech, delusional manifestation and flight of ideas. His uncle
told that he had stayed up all night and ran naked down his street. He was offered quetiapine 100 mg and diazepam 20 mg Commented [OET5]: reported
but he refused. Local mental health team was updated and he was accompanied with his uncle.
