MASSIVE FLUE VACCINATION OF GRADES FOUR TO SIX PUPILS

NARRATIVE REPORT

The massive flue vaccination of grades four to six pupils of Dakok Tamulon Integrated
School was held at 8:00 0’clock in the morning on December 16, 2021(Thursday) at DTIS’
gymnasium. The objective of this activity is to help safeguard children by making sure they are
protected from the vaccine-preventable disease.
The signing of parent consent attendance started at exactly 7:30 in the morning and
was followed by a simple program consists of opening prayer by Mrs. Marlyn U. Manaya,
Grade -2 Love adviser; welcome address by Mrs. Lilibeth H. Panizales, School Health and
Nutrition Coordinator; the vaccine orientation by Ms. Mary Rose Wenna Ea, East Malungon
District Nurse; Inspirational Message by sir Adriano A. Daligdig , East District Supervisor; and
message of Hon. Ramil T.Rodrigo, Barangay Counselor at the same time the DTIS’ SGC
President.
The vaccination started after all parents signed the parents’ consent. There were ____
pupils successfully vaccinated on that day.
