1.0 PURPOSE

The purpose of this method statement is to identify the requirements for concrete work. This
method statement covers the general method for reinforcement re-bar, anchor bolt/
embedded plate installation works, formworks, concreting works and curing within this project.

2.0 SCOPE

The Scope of this procedure is to cover & define the minimum requirements to be considered
for the workmanship associated with concrete works for the 07 Nos of Wellhead Compressors
(each having capacity of 60 MMSCFD) project, location-A of Titas Gas Field. This document
shall include preparation of material, manpower and equipment, construction, inspection and
test foundation.

The following is our company organogram.

5.2 EQUIPMENT

a) CONCRETE MIXING: Plant Generally, we will use RMC, if required or manual


mixing concrete for foundation works and have planned for manual mixing
concrete for PCC.

b) WELDING GENERATOR & WELDING MACHINE: Welding generator,


independent of main electric supply can be used to operate welding machine to
weld any equipment whenever required.

c) WATER PUMP: The water is pumped from wells or sumps to temporarily lower
the groundwater levels, to allow excavation in dry and stable conditions below
natural groundwater level.

d) VIBRATOR: Vibrating is significant on many concrete construction projects.


When concrete is poured, it might have hundreds or even thousands of air
bubbles, which can substantially weaken the concrete structure. Concrete

vibrators eliminate air bubbles by vigorously shaking the freshly poured


concrete.

Face Masks, Ear Plugs, Hand Gloves, Safety Belts, Hand Sanitizer etc.

6.0 RAW MATERIALS

6.1 CEMENT

a) Considering high heat hydration of Ordinary Portland Cement (OPC) specially


apply in mass concrete, there is a quickly increasing of heat hydration in early
period. It is not easy to dissipation due to heat hydration of mass cement.
Temperature difference between surface and inside of concrete is large so that
there are harmful cracks. Therefore, it shall use Ordinary Portland cement to
ASTM C150 (Type I) which has a slower increasing of heat hydration in early
period, shall be used in all concrete works.

b) Cement shall be stored in weatherproof sheds and used in chronological order


such that no bags are held in storage for more than 4 months. Cement passing
this date shall not be used on this Contract. Any cement which has become
contaminated or has deteriorated shall not be used in the Works. A
manufacturer's test certificate shall be submitted by the Contractor to the
Employer on the day of delivery of the cement.

6.2 RE-BAR

The steel of all types of reinforcement shall conform in all respects with the standard
practices of ACI/BNBC/ASTM as applicable in oil & gas plant construction. Certificates
of conformance from the steel mills shall be presented to the Engineer with each
delivery by the Contractor.

All re-bar shall be bent in cold. Heating of bars of purpose shall be prohibited.
Deformed bar shall be comfort to design drawing which fy = 420 N/mm2.

Fusion bonded epoxy coated ASTM A615 GRADE 60 reinforcement bar is to be using
in all structure.

The steel shall be free from oil, dirt, paint, loose rust, mill scale or other deleterious
matter immediately before placing the concrete.

6.3 COARSE AGGREGATE

It shall employ crushed stone which particle size is 20 mm. It can reduce usage of
cement, thus, decrease hydration heat of cement and temperature rise rate of
concrete.

Coarse aggregate stored on the site on clean platforms/ Area free from direct contact
with the ground and in separate bins or batches for each size. Sufficient time must be
allowed for storage of the aggregates on site to facilitate the drainage of excess
water. Grading requirement for coarse aggregates shall comply with below Table 3 of
ASTM C33.

6.4 FINE AGGREGATE

Fine aggregate shall consist of natural sand as approved by COMPANY. The sand for
concrete shall be capable of passing through a 5mm ASTM test sieve and it shall be
so graded that when mixed with the coarse aggregate and cement it shall produce a
concrete of the maximum density. Crushed sand may be added to natural sand in
proportions approved by the COMPANY. in order to achieve the required grading.
Crushed sand alone may only be used with the approval of the COMPANY. Fine
aggregate shall be graded within the following limits in Table 7-3 below, and according
with Section 6 of ASTM C33.

6.5 WATER

Water used for cleaning aggregates and for mixing and curing of concrete works shall
be obtained from an approved source and shall be clean and free of acids, oils,
vegetable and deleterious matter, which may have an effect on the strength or normal
Curing time and appearance of the hardened concrete by discoloration or
efflorescence.
Water used for mixing with concrete, mortar and grout shall be in accordance with ACI
318 / ASTM C94. Water used for curing, or any other function relating to placement of
concrete shall be potable.
Wash water from mixer washout operations shall not be used as mixing water. The pH
value of water shall be within the range of 6.0 to 8.0.
In hot and cold climates, the water shall be chilled or heated water respectively in
accordance with ACI 305R & ACI 306R.
Sea water shall not be used in any concrete related works or mixed with approved
potable water designated for making of concrete.

6.6 FORMWORK MATERIALS

All formwork and supports shall be of suitable quality of such dimensions and strength
and fixed in a manner that will ensure that they remain rigid during the placing,
compaction and setting of the concrete. All joints on formwork shall be close fitting so
that there will be no leakage of cement grout through the joints. All joints shall be
either horizontal or vertical in a uniform pattern. Formwork shall be of such material as
to ensure a true smooth surface on finished structural concrete and all exposed
corners above ground shall have a 25 mm chamfer.

7.0 WORK PROCEDURE


7.1 SITE PREPARATION
a) Only qualified people are permitted to enter the work site area.

b) All equipment will be inspected for conformity to the requirements of the work
place according to Safety and Health Act. and specification prior to deployment
to the project site.

c) Inspections shall also be conducted to determine that in the case of a specialist


operator, the person is duly licensed by the appropriate authority to operate
such equipment. Appropriate test certificates and legal documents as per
requirements, including insurance, shall be submitted to Contractor’s HSE
department upon the equipment mobilization. In addition, equipment
maintenance required to be maintained by competent person.

7.2 LEAN CONCRETE WORKS

a) Surveyor shall mark the lines and levels of the area for lean concrete according
to contractor’s appearance and with reference to temporary and reference
bench marks.
b) Final formations level will be tested according to the requirement and visually
checked by QC Engineer. The surface will be tested according to the
requirement and visually checked and should be with ITP, thereafter clearance
shall be given to commence blinding works.
c) Formworks is set out and constructed to the required dimensions, location and
elevation as per IFC drawing. The blinding concrete will be compacted, levelled
off and trowel finished to the correct elevation and cured to the specified
requirements.
d) The actual level of soil shall be clearly identified, and the depth of blinding shall
be reflected on the inspection report.
e) Top level of blinding concrete to be marked. It will be concreted and treated as
guiding levels during concrete pour.
f) Blinding concrete shall be as per approved Mix design.
g) The minimum thickness of the blinding concrete shall be following as standard
drawings unless otherwise shown on drawings.

7.3 REINFORCEMENT WORK

Re-bar shall process in prefabricate yard. It shall use manual or machinery equipment
to cut and bend rebar. For re-bar welding, it shall be manual and double-welded.
Concrete cover of rebar will use mortar precast block or finish product protect block.

❖ Preparation
a) It shall execute charger sheet audit policy strictly. There shall be sign by
rebar foreman on charger sheet, and then it shall be issued.

b) Material quality is one of the basic conditions to ensure rebar work


quality. All rebar must be with certificate which lists all information.
Rebar cannot enter to site without certificate. According to relevant
requirement, entered rebar shall be sampling inspected by batch, and
accepted rebar shall be used.

c) Constructor level shall carry out explanation system. Rebar foreman


shall organize technical explanation before setting out and colligation. It
shall let worker know and construct as specification and solve key
construction problem.

❖ Rebar quality requirement and stacking

Rebar on-site shall have quality certificate: There shall be mark on surface of
each bundle of rebar. It shall check mark and surface onsite; greasy dirt shall
be clean up before using. Based on relevant specification, it shall take sample
to test mechanical property under witness of owner. After retest accepted, it
shall be processed. Rebar onsite shall be marked identification and placed in
different area based on specification.

❖ Rebar Processing

a) During cutting progress, it must be cut if there is any rebar with split or
seriously elbow. If hardness and type of rebar are quite different, it shall
report immediately, it shall continue to process after ascertain cause.

b) Semi-finished rebar shall be applied inspection and acceptance system


in whole processing. It will process in bunch after accepted the first
finished rebar. It is key point to check that kind of rebar which are large
amount, difficult to be processed and has a great effect on colligation.

c) Finished and semi-finished rebar shall be managed by special assigned


person after accepted. It shall be marked identification and placed in
different area. It will give out according to demand.

❖ Rebar Welding

All rebar welding use on this project is electrode welding.

Double side lap welding can be used for the base plate of rebar which lap
length is 5d and weld width is greater than 0.7d,Welding quality shall conform
to requirements of specification.
a) It shall check power, welder, equipment and tool. To prevent rebar burnt
by arc starting, it shall ensure welding ground wire be strung well.
b) For selecting welding parameters, it shall base on grade, diameter and
joint type of rebar and welding location. In order to ensure good fusion
between weld seam and rebar, it should select appropriate diameter of
welding rod, welding layer and welding current.
c) It will test welding and make simulated specimens. It shall weld 3
simulated specimens to make tensile test. It will weld large amount in
certain parameter after inspection accepted.

❖ Rebar colligation and installation

a) It need consider sufficiently about construction condition while setting


out. To avoid hammering and prying violently onsite, it needs rationally
arrange space between rebar and rebar, rebar and embedded ironware,
rebar and installed embedded part.

b) Rebar process shall base on cutting list. Cutting list notes usage part,
specification, species, hook form, bending radius and other
requirements. It can be colligated after chalked rebar location line.
Before pouring concrete, it shall ensure that surface of rebar is clean,
without sludge and so forth.
c) Anchorage and lap length of rebar shall conform to requirements.
d) Requirements of hook: When rebar need to hook as 90° or 45°,
bending of rebar shall be done accordance with design requirements
(CPTA-CCJV-10-STR-DWG-0007_Rev-1, CPTA-CCJV-10-STR-DWG-
0008_Rev-1, CPTA-CCJV-42-STR-DWG-0004_Rev-0).

❖ The rebar shall be installed as per the design drawing and bar bending
schedule.
❖ Concrete spacers shall be installed with proper spacing in order to carry out the
load of the rebar and embedded item.
❖ Reinforcement of columns/pedestals to follow the accurate markings on the
blinding concrete.
❖ rebar shall be placed in the correct line & level as indicated in the approved
structural drawing.
❖ Spacers and chairs shall be provided to maintain the specified covers. Interval
as to maintain the top and bottom covers, or as indicated on the approved
structural detail drawing.

❖ All the service / sleeves and embedded items shall be in compliance with
approved drawings.
❖ After completion of installation of reinforcement steel all contaminants shall be
removed from the concrete pouring area.
❖ Inspection shall be based on approved ITP provided.

7.4 FORMWORKS
a) Form-work for all foundation concrete shall be made from plywood, timber, steel
and supporting steel wires. The design and use of form-work shall be in
accordance with design requirement.
b) The formwork shall be erected by providing sufficient props and struts including
bracing, using scaffolding and staging arrangements, where necessary.
c) The surveyor shall mark on the protection board above the blinding the outer
formwork to fix along the perimeter of the foundations. Accurate alignment, line,
level, shall follow as per drawings.
d) Faces of formwork in contact with concrete shall be free from adhering foreign
material, projecting nails and, splits or other defects, and all formwork shall be
clean and from standing water, dirt, shaving, chippings or other deleterious matter.
e) Formwork shall be strong enough, connected and stiffened so as to avoid their
displacement and disconnection during pouring & vibration. All forms will be set
and maintained in their correct position and shape so that final concrete structure
is within the tolerance limits as per specification of design requirement.
f) The form-work shall be arranged as to be readily dismantled and removed from
cast concrete without any shock, disturbance or damage.
g) Releasing/ Stripping agents shall be applied to the surface of the form work as
recommended by the manufacturer.
h) Required anchors dowel bars pipe sleeves and other inserts shall be provided as
per the Approved structural drawings.
i) Approved water stopper shall be provided in construction joint for the structures /
structural part as per drawing. Lines, levels and dimensions of the form-works shall
be checked before placing concrete.
j) All the concrete finish levels shall be marked on the formwork by suitable manner.
k) The deviation of the dimensions and elevation to the IFC drawings shall to be
according to the acceptable tolerance as per standard drawing.
l) Inspection shall be carried out as per ITP provided.

7.5 INSTALLATION OF PIPES, ANCHORS BOLTS AND INSERT PLATES

a) Additional reinforcement shall be installed around the pipe sleeves as per design
drawings.
b) Appropriate thickness of pads or as described in design drawings shall be provided
around the pipes with specific details for the joint between pipes and concrete.
c) The insert plates and anchor bolts shall be installed to the correct sizes, line, level
and spacing as per approved drawings CPTA-CCJV-10-STR-DWG-0008_Rev-1.
d) Bolt threads, exposed surfaces of partially embedded items shall be
protected by covering it with masking tape or, suitable material to protect
contamination from concrete pour.

e) The insert plates and anchor bolts shall be tightly secured in order to avoid any
displacement during concreting works.
f) Level and line for each insert shall be checked and secured correctly before
placing of concrete.
g) Inspection shall be carried out as per ITP provided.

7.6 TRANSPORTATION AND PLACING OF CONCRETE

7.6.1 TRANSPORTATION

Transportation shall be as rapid as is practicable and by methods which


exclude segregation or loss of components.

If transportation is done by chuting, the flow shall be continuous. The Acute


length shall not exceed 5 meters and its delivery end shall be as close as
possible to the point of deposit. In the case of intermittent operation, the spout
shall discharge into a hopper.

Transportation and pouring of the concrete shall not exceed 30 minutes.

7.6.2 DELIVERY

Discharge of the concrete shall be completed within 45 minutes after the


introduction of the mixing water to the cement and aggregates or the
introduction of the cement to the aggregates (this time may be extended up to
90 minutes with approval of the Company).
Re-tempering shall not be permitted. Re-tempering is defined as the addition of
water to restore slump lost during excessive mixing due to excessive lapse of
time since initial mixing;
Concrete delivery tickets shall be maintained as a record for ready mixed
concrete.
Equipment for conveying concrete, such as buckets, trucks, belt conveyors,
pumps, etc., shall be of such design, size and condition to ensure a continuous
and adequate supply of concrete of the specified mix and slump, without
segregation at the point of deposition.

7.6.3 PLACEMENT OF CONCRETE

Before placement, all equipment for delivery and placing shall be cleaned and
all debris. loose or unsound material, water and ice removed from areas to be
concreted.
Concrete shall not be placed against frozen ground or sub grade, nor against
soil which has been subject to excessive water or drying; The minimum
temperature of concrete during placing shall be 10°C. The maximum
temperature shall be 35°C.
Concrete placement shall be such as to prevent water from collecting at the
ends and corners of the forms and along form faces.

Concrete shall not be poured during rain unless adequately protected; Vibrating
of concrete shall conform to ACI 301.
Special care shall be exercised to ensure vibrators do not interfere with
reinforcing steel or form work. One spare, workable vibrator shall be
maintained at all times while concrete is being placed.
Inspection and authorization shall be obtained from the Company before
placing concrete.
Concrete that has achieved initial set or has been contaminated by foreign
matter shall not be deposited in the structure.
Materials and equipment for protection and curing shall be operational at the
placement site before placement begins.
The interval between concrete deliveries shall be such that no more than 20
minutes interruption elapses during placement of a single foundation or slab,
unless approved by the Company.
Mass Concreting shall be done as per ACI 207.1R, ACI 207.2R and
ACI207.4R.
Handling and placing of concrete shall comply with ACI 304R requirements:
Free fall of concrete shall be kept to a minimum and shall not exceed 1.5 m.
The CONTRACTOR shall use suitable chutes, baffles and trunks to confine
and control the falling concrete;
For placing by pumping the concrete mix shall be in accordance with ACI
304.2R.

7.6.4 COMPACTION

Compaction of fresh concrete shall be undertaken as per ACI 309-R.


Full compaction of the concrete shall be achieved throughout the entire depth
of the layer. It shall be thoroughly worked against the formwork and around the
reinforcement and successive layers shall be thoroughly bonded together. Air
bubbles formed during the mixing and casting shall be expelled and particular
care shall be taken where sloping soffits occur.
Unless directed otherwise, approved power-driven poker vibrators shall be
used for compaction. Internal vibrators used shall be the largest size and the
most powerful that can be used properly in the work. The vibrator head
diameter shall be not less than 50 mm for walls, columns, beams and slabs.
Use of vibrators to transport concrete within forms shall not be allowed.
The radius of influence shall be at least 300 mm.
Vibrators shall be inserted vertically at such distances apart, or applied in such
a manner, as will ensure that the concrete is satisfactorily and uniformly
compacted. Immersion vibrators shall penetrate the full depth of the layer, and
when the underlying layer is of fresh concrete, shall enter and re-vibrate that
layer to ensure that successive layers are bonded together. Over-vibration
causing segregation, surface laitance or leakage through formwork shall be
avoided. Immersion vibrators shall be withdrawn slowly to prevent the
formation of voids. Vibrators shall not be used to work the concrete along the
forms, or in such a way as to segregate the mix, damage formwork or other
parts of the work, or displace the reinforcement. External vibrators shall not be

used without approval being obtained for the formwork design and the
configuration of the vibrators.

7.7 CONSTRUCTION JOINTS

When concrete pouring is done between column & beam / girder / wall joints,
pouring of concrete shall be delayed to ensure that the concrete in column
portion is no longer plastic.
Saw cuts for control joints shall be made within 12 hours after concrete is
poured.
Control joints shall be cut in accordance with the saw manufacturer’s written
recommendations. Sawing sequence shall be based on pour time and size of
slab.
Kickers shall be cast monolithically with beams/slabs/foundations. They shall
generally be positioned at 150 mm above slabs and bases for walls and
columns (Kickers) and may also be required because of limitations of
concreting or shuttering.
Construction joints shall be positioned as shown on the drawings and shall not
be altered. Construction joints shall be made by forming the concrete on one
side of the joint and allowing it to set for at least three days before concrete is
placed on the other side. The location of additional construction joints and
sequence of placements shall be subject to the approval of the COMPANY and
the CONTRACTOR shall make all necessary adjustments to the reinforcement
and seals to suit approved joints and the sequence of placements. The joint
surface should be roughened to remove laitance without disturbing the coarse
aggregate by pressure jetting with air and water or wire brushing. Care shall be
taken to ensure that the joint is clean prior to placing fresh concrete.
Unless otherwise shown on the drawings, expansion joints shall be made by
forming the concrete on one side of the joint and separating it from the
concrete on the other side by the use of joint filers. With the exception of dowel
bars in floor slabs, other metalwork or reinforcement shall not extend through
expansion joints.
Expansion joints shall generally be positioned at every 20m and contraction
joints shall be at every 6m. However, spacing of joints to be calculated and
embedded reinforcement quantity adjusted to suit requirements.
Isolation joints 20 mm thick shall be placed where the pavement adjoins
vertical surfaces (e.g., walls, columns, catch basins, manholes, and equipment
foundations). Isolation joints shall be located in accordance with the Contract
Documents. These joints shall be sealed with an approved jointing material and
flexible sealant.
Dowels at expansion joints shall be properly aligned to prevent any restraint on
expansion movement at the joint.

7.8 CONCRETING

a) Concrete will normally be deposited in layers not greater than 300mm. The layer
should be as level as possible so that vibrator does not need to move the concrete
laterally that might cause segregation.
b) Submit Request for Inspection (RFI) to the Quality department indicating schedule
date/time of concrete pouring. Ensure that all areas schedule to receive finishes
have been inspected and are acceptable prior to pouring.
c) Prior to concrete pouring, the rebar and formwork shall be cleaned to ensure no
contamination along the rebar installed and form works erected.
d) After carrying out cleaning, visual inspection shall be carried out to ensure that the
area is clear and ready for carrying out concrete.
e) Top level of concrete shall be marked properly on the form-work as guide levels for
the required elevation during concreting shall be maintained with appropriate
supervision of respective site engineer.
f) Adequate staging with working platforms shall be provided for compacting/
vibration and supervision. Ensure that areas/ structures schedule for pouring have
been inspected and are accepted for pouring.
g) A pour card will maintain for each location of concrete.
h) Concrete drop must not be higher than 1.5m to avoid concrete segregation.
Concrete pouring is to be carried out at such a rate that it remains plastic and flows
readily into spaces between reinforcement.
i) Concreting shall be carried out as a continuous operation until placing of a panel or
section as defined its boundaries. Or pre-determined joints that are made in a
location that will not impair the strength of structure.
j) Concrete shall be compacted by vibration during the operation of placing around
the reinforcement, around embedded fixtures and into corners or the formwork to
form a solid mass free from voids.
k) The vibrators shall be operated at regular intervals to produce dense, compact,
homogeneous concrete surface. The size of the vibrating needle shall be decided
considering the layout of reinforcement and shall ensure consistent mass is
obtained without segregation. The diameter of the vibrator needle for Compressor
foundation shall be 2 inchs for foundation works, column beam and 1.5 for footing
and shall ensure thorough compaction of the entire volume of the concrete, Form
vibrators can be used for vibration.
l) Vibrators shall be withdrawn slowly to prevent the formation of voids and shall not
be placed against compacted concrete. The vibrations transmitted to the formwork
can loosen wedges and fixings. We shall ensure to have a close watch on all
fastenings as it is necessary to avoid loosening. Similarly, wedges shall be
regularly checked and tightened.
m) Adequate and not excessive compaction of concrete through vibration shall be
observed to avoid any segregation of the poured concrete.

7.9 CONCRETE TESTING

Sampling rates of fresh concrete shall generally be based on frequency and volume
poured. Two (02) sets cylinder sample shall be taken in each batch from 38 m3

concrete volume in which 01 (one) set should be tested for 7 days and another 01
(one) set for 28 days. The test shall establish the quality of the concrete and the
probable 28-day strength.

All cylinder shall be marked and dated for identification of the section poured and the
Contractor shall submit copies of all cylinder tests performed showing the following: -

➢ Cylinder number.
➢ Date and time sampled.
➢ Location of concrete placing within the Works.
➢ Date and time of test.
➢ Crushing strength.

A comprehensive statistical check of this information shall be made on a regular basis


during concreting works.

7.10 CURING

Concrete shall be cured and protected in accordance with ACI 308R.


During the curing period only, such areas as are needed for immediate surface finish
treatment shall be uncovered. After finish treatment, such areas shall be immediately
re-covered, and the curing continued.
Sealing curing compounds shall not be used unless specifically approved by
COMPANY and to be used strictly in accordance with manufacturer’s
recommendations.
Concrete not in contact with forms may utilize any of the curing methods indicated in
ACI 301, for the preservation of moisture. However, for interior concrete slabs, ponding,
sprayed water, or wet sand shall not be permitted.
Unless otherwise specified in the Contract Documents, liquid membrane curing
compounds shall not be used on surfaces that shall receive bonded treatments, tiles,
paint or other coatings, epoxy toppings, or additional concrete.
Regular and frequent inspection shall be made of concrete being cured and tests shall
be made on the permeability of cured concrete which will establish whether or not
curing has been correctly carried out.
All water used for curing shall be of the quality specified in Section 7.8.
The exposed surfaces of the concrete shall be protected during hardening from the
effects of sunshine, drying winds, rain or running water.
All concrete shall be cured for a period of time required to obtain the full specified
strength, but not less than seven (7) consecutive days;
Curing of the concrete after the initial set shall be by one of following methods
subjected to acceptance:
- Ponding with water including protection from atmospheric contaminations
- Application of absorptive mats of fabric or hessian kept continuously wet and covered
with sealed polyethylene sheet
- Curing compounds may be used in specific approved situations as given hereunder.

8.0 QUALITY MANAGEMENT

8.1 INSPECTION AND TEST PLAN

All stage of works shall be inspected in accordance with approved Inspection and Test
Plan provided as attachment.

8.2 QUALITY CONTROL MEASURE

8.2.1 It should apply technical responsibility system in construction site. Leading


by engineer, organize technical leader team by constructor, surveyor,
documenter and each foreman.
8.2.2 Drawing, technical, operation and acceptance specification shall be
learned.
8.2.3 It should audit drawing and make notes, if there is any unclear detail, it
shall be communicated with responsible engineer immediately.
8.2.4 Prepare re-bar setting out, bill of material and each material plan in time.
8.2.5 Organize technical explanation in time and process handover in advance.
8.2.6 All survey and measure equipment used in this project must conform to
relative requirement.
8.2.7 Concrete supplier should provide mix proportion report to contractor and
material inspection report which include sand, crushed stone, cement and
additive will be submitted
8.2.8 Each construction process is critical. Each foreman is taking primary
responsibility for his task, it will keep strictly rules for reward and
punishment. Technician should make technical explanation for each
process.
8.2.9 It shall follow construction procedure strictly to solve common problem in
constructing.
8.2.10 Each foreman should get hold of quality issue in time during constructing
period. If any problem, found should be solved in time.
8.2.11 Onsite specific quality inspector should know drawing, operation
specification and acceptance specification well; check finished process of
each construction team and correct quality problem in time then report it.
Re-work or rectified activity shall be executed double check policy. It will
start next process after re-work conformed to requirement of design and
acceptance specification.
8.2.12 In order to ensure that construction quality is controlled efficiently; process
handover and inspection policy shall be executed strictly. Operator should
self-inspect for his process, and next processing operator will inspect for
previous process. It shall be process handover after accepted by self and
mutual inspection.
8.2.13 It shall enhance to inspect purchase order and identification of product,
control process and complete all records.

9.0 HEALTH, SAFETY & SECURITY

9.1 SAFETY PRECAUTIONS

The work will be executed in accordance to the requirements under the local legal
requirements, In House Regulation, Construction HSE Instruction for the Sub-
Contractors.

All personnel to be provided and equipped the minimum PPE during their work;

• Safety helmet with chin strap / Company Logo


• Safety shoes
• Eye protection
• Safety gloves to be worn

Restriction for the access of all third parties will be implemented to avoid any incident
during work. With regard to the overall safety control plan, all the works will be carried
out following the established/approved HSE Plan in Brief.

9.2 SAFETY REQUIREMENTS


• All operations in connection with pile load testing shall be carried out in such a
manner so as to minimize, avoid, or eliminate the exposure of people to
hazard. The following safety rules are in addition to general safety
requirements applicable to construction operations:

• Keep all test and adjacent work areas, walkways, platforms, etc. clear of scrap,
debris, small tools, and mud, grease, oil, or other slippery substances.

• Hydraulic jacks shall be equipped with spherical bearing plates or shall be in


complete and firm contact with the bearing surfaces and shall be aligned so as
to avoid eccentric loading.

• Loads shall not be hoisted, swung, or suspended over anyone and shall be
controlled by tag lines.

• The test beam(s), reaction frame, and other anchoring devices, test boxes, and
their connections and supports shall be designed and approved by an engineer
and installed to transmit the required loads with an adequate factor of safety.

• All test beams, reaction frames, platforms, and boxes shall be adequately
supported at all times.
• All reaction loads shall be stable and balanced. When using loading method,
safety wedges shall be in place at all times to prevent the platform from tipping.
During testing, movements of the reaction load or system should be monitored
to detect impending unstable conditions.

• Only authorized personnel shall be permitted within the immediate test area,
and only as necessary to monitor test equipment. As best as possible, test
monitoring equipment at a safe distance away from jacks, loaded beams,
weighted boxes, dead weights, and their supports and connections.

9.4 SAFETY PROVISIONS

9.4.1 Movement of heavy equipment shall be done under close supervision of


highly skill person.

9.4.2 All trailers and truck mixture used for piling work must be driven carefully

9.4.3 All vehicles are to drive within the site approved speed limit (15 km/hrs.).

9.4.4 All site personnel who involve in the unloading of machinery and materials
must wear minimum required PPE, like safety helmet, safety vest, safety
gloves and safety shoes.

9.4.5 The Supervisor will ensure that all tools and equipment, including the
appropriate PPE, is available and in good condition.

9.4.6 Hard barricade has been applied for existing properties and close
Monitoring will be done.

9.4.7 Maintain safe distance while working.

9.4.8 Stable access shall be prepared if necessary.

9.4.9 All persons entering the construction areas must be induced in accordance
with the safety plan procedure.

9.4.10 Cutting piles and rebar must be carefully operated. Gloves must be used
during pile cut-off.

9.4.11 All operatives must wear safety belts during working at height.

9.4.12 All operatives being outdoors must stop working during heavy raining and
lightning periods.

9.4.13 Defective equipment must be properly rectified or not to be used.

9.4.14 For COVID-19 protection, all operatives should wear mask, use hand
sanitizer or frequent hand washing.
