PROFESSIONAL EXPERIENCES:
 I have worked as EPI Officer in Parwan Province from 1/01/2022 up to now with
JACK Organization.
 I have worked as covid 19 EPI supervisor in Bamyan Province with AKHS
organization from 12/08/2021 up 30/09/2021.
 I have worked as covid 19 focal point in Daikundi Province with Move BPHS
project from 30/12/2020 to 20/06/2021.
 EPI Officer& CBoV focal point in Daikundi Province BPHS project with Move
Welfare Organization from 23July 2018 up to 08 August 2021.
Main Responsibilities:
 Responsible for planning, supervision, monitoring, implementation and follow- up EPI
components in all health facilities and outreach program, Fix and Mobile activities (close
follow up of each vaccinator performance toward the target)
 Coordinate with PEMT and UNICEF to ensure vaccines/supplies are delivered to the
health facilities on time.

Health services for TSFP for PLW and 6-59 month of children.
 Develop capacity building /training plan for TSFP field staff (Nutrition nurse/stock
keeper)
 Community Development Mobilizer in Bamyan Province with
AADA/UNFPA/FHH project from 1st Jan 2011 to 1 January 2012.
Main Responsibilities:
 Responsible for planning, implementation, supervision and monitoring the CBHC
related activities in all FHH catchment areas. Maintain/update CHWs database at PO
level.
 Conduct training for CHWs regarding CBHC program and assist in developing
capacity building plan in collaboration with provincial technical manager.
 Ensure Regular supply and F/up of CHWs kits initial and re supply through assigning of
CHS and F/up of consumption.
 Link community with the FHH through health shuras and their active participation.
 Ensure accurate recording and reporting system is maintaining by HPs and timely
collection.

 Community Development Mobilizer in Bamyan Province with


AADA/UNFPA/FHH project from 1st Jan 2011 to 1the Jan 2012 (One year)
Main Responsibilities:
 Conducted regular meetings with community leaders regarding community mobilizing
for increasing project all indicators coverage.

 Liaise the project with community through regular participation in community events,
sharing of information about project targets, involvement of the community elders,
referral system etc...

Main Responsibilities:
 Responsible for planning, implementation, supervision and follow up EPI service for
under catchments area population of FHHs through MSTs.
 Ensure proper supply of routine vaccines and cold chain equipment for all MST of FHH
program.
 Provide technical support, in-service and on the job training for vaccinators regarding
EPI.
 Ensure reporting and recording system of EPI service delivery at all FHHs, timely
collection, analyze and interpretation of data as well as updating EPI database on monthly
basis.
