School Nurse Document Control Staffer Quality Management Representative

1.0 Objectives:

A. Check for possible diseases so they can be treated early.


B. Identify any issues that may become medical concerns in the future.
C. Update necessary Immunizations.
D. Ensure that the employee is maintaining a healthy diet and exercise
routine.

2.0 Scope:
This is a procedure that the Health Services staff will perform to check the
overall health. The Healthcare Staff is the School Nurse and the University
Physician. It will help determine the general status of employee’s health.

Notes:

Implementing Guidelines

1. Employees will undergo the schedule of Annual Physical Examination


First quarter of the school year. This will include at least the following
examination:

● Chest X-ray
● Complete Blood Count

2. Health Services will coordinate with the approved Laboratory for


conduct of
examination.

3. Health Services is in charge of getting the results from the affiliated


laboratory and will record all findings in the individual medical records.

4. Health Services will set an appointment date for Physical examination


with the employee to the University Physician.

5. Employees will fill out an Individual Health Assessment Form that will
be provided by the Health Services.

6. On the actual date of examination to be performed at the Medical Clinic,


the University Physician will point out ​any findings and will advise the
patient on what to do.

Based on the initial assessment findings, employees concerned may need


laboratory work ups and will submit results before the final medical
assessment will be done by a University Physician.

7. An Average physical examination may include the following:

a. Updated health history


b. Vital Signs monitoring
c. Visual examination
d. Physical exams and Dental Examination
e. Laboratory tests

8. Dental examination with the University Dentist will be scheduled through


the Health Services Nurses for employees and students.

9. The University Physician may request a return visit to discuss test results
or follow
up on any examination findings

10. If no additional tests are needed and no problems arise, the employee is
set to do annual Physical and medical examinations until next year.

11. Employees medical assessment will be classified according to:

Class A. Physically fit for any work

Class B. Physically underdeveloped or with correctible defects (error of refraction, dental


caries, defective hearing, and other similar defects) but otherwise fit to Work

Class C. Employable but owing to certain impairments or conditions (heart disease,


hypertension, anatomical defects)

Requires special placement or limited duty in a specified or selected assignment


requiring follow-up treatment/periodic evaluation.

Class D. Unfit or unsafe for any type of employment (Active PTB, advance heart disease
with threatened failure, malignant hypertension, and other similar illnesses)

12. Results of Physical Examination of employees will be endorsed to the


Human Resource Department.

● Mental Health referrals will be endorsed by Health Services to


Guidance, Testing
and Wellness Center(GTWC)

13. Medical findings which will need sick leave, Health Services will fill out
necessary forms needed
which includes SSS Sick Leave Form or Employees Compensation form
for work related accidents/illness.

14. SEPARATION FROM EMPLOYMENT - An employee leaving the


employment of the University shall, if necessary, be examined by the
University Physician:
a.To determine if the employee is suffering from any occupational
disease;
b.To determine whether he is suffering from any injury or illness
which
has not completely healed
c.To determine whether he has sustained an injury.

15. TRANSFER EXAMINATIONS- applicants examined for employment


and
accepted for specific work or job shall not be transferred to another work
or job until they have been examined by the University Physician and
certified that the transfer is medically advisable.

16. Health Services monitors all Physical examination results and serves as a
baseline for Health promotion/health education programs.
